摘要
目的探讨糖尿病肾病(DN)患者外周血中基质细胞衍生因子-1(SDF-1)水平的变化及意义。方法测定74例2型糖尿病(DM)患者和26例健康对照者(NC组)外周血中SDF-1水平、内皮祖细胞(EPC)数量、EPC表面SDF-1的受体CXCR4表达率。DM患者按尿白蛋白排泄率分单纯DM组(SDM组)、早期DN组(EDN组)、临床DN组(CDN组)。结果SDF-1水平和EPC数量NC组、SDM组、EDN组、CDN组依次下降(P<0.05或0.01);CXCR4表达率SDM组较另3组升高(P<0.05或0.01),CDN组较其它组降低(P<0.05或0.01);2型DM患者SDF-1与HBA1c、病程、收缩期血压负相关(r=-0.398、-0.257、-0.241,P<0.05或0.01),SDF-1与EPC正相关(r=0.278,P<0.05);UAER与SDF-1、EPC负相关(r=-0.263、-0.248,P<0.05),UAER为因变量多元逐步回归分析,进入方程的变量依次为HBA1c、收缩压、SDF-1、EPC和病程。结论2型DM患者外周血中SDF-1/CXCR4轴的变化与DN的发生、发展有关。
【Objective】To study the changes of serum stromal cell-derived factor-1 (SDF-1) in patients with diabetic nephropathy (DN) and its implication.【Methods】74 cases patients with type 2 diabetes mellitus were divided into simple diabetic group (25 cases),early diabetic nephropathy group (25 cases) and clinical diabetic nephropathy group (24 cases) according to their urinary albumin excretion rate,a total of 26 healthy people were enrolled as control group. The level of serum SDF-1,the positive rate of CXCR4 dwelling on the membrance of endothelial progenitor cells (EPC) from peripheral blood and the number of EPC were determined. 【Results】The serum SDF-1 levels and the number of EPC were decreased by degrees from the control to simple diabetic,early diabetic nephropathy,clinical diabetic nephropathy groups (P〈0.05 or 0.01). Compared with the other groups,the positive rate of CXCR4 in the simple diabetic group was increased (P〈0.05 or 0.01),which in clinical diabetic nephropathy group is lower than in the other groups (P〈0.05). There was a significantly positive correlation between the level of SDF-1 and the number of EPC (r =0.278,P〈0.05). HBA1c,SDF-1,EPC,systolic blood pressure and duration were correlated with urinary albumin excretion rate independently. 【Conclusion】SDF-1/CXCR4 may be related to diabetic nephropathy.
