摘要
目的探讨影响脑性瘫痪(CP)患儿平地步行速度的影响因素。方法选取佳木斯大学附属第三医院30例CP患儿作为CP组(男15例,女15例)。同时选取佳木斯某小学30名健康儿童为健康对照组(男15名,女15名)。通过足印步态分析及计算机视频技术测量、记录CP患儿及健康儿童的快适速度和最大速度步行时资料,比较同一组儿童在不同行走状态时步频、步幅之间的差异,同时比较2组儿童在不同行走状态时行走速度、步频、步幅之间的差异,分析影响CP患儿步行功能的主要因素,为临床制定治疗方案提供依据。结果与健康对照组儿童相比,CP组患儿无论在快适速度还是在最大速度步行时步行速度明显降低,步幅减小,而步频差异无统计学意义;随着步行速度增加,健康对照组儿童在单位时间内行走的步数(步频)及步幅均显著增加,而CP组患儿仅步频随着步行速度增加而增大,步幅未出现明显增加。结论对于CP患儿来说,步幅可能是影响步行速度的重要因素之一,扩大患儿步幅是改善患儿行走能力的重要方式。
Objective To investigate the factors of flat walking velocity in children with cerebral palsy (CP). Methods Thirty patients (15 male, 15 female) were selected randomly from the 3^rd Affiliated Hospital of Jiamusi University for CP group,and 30 healthy children( 15 male, 15 female) were selected randomly from a primary school of Jiamusi for healthy control group. Foot printing and video technique of com- puter were used to measure, record the comfortable walking velocity and the maximum walking velocity in 2 groups. Compared with the stride frequency and the stride length when children in different walking states. Compared with the walking velocity, stride frequency and the stride length between 2 groups. The factors associated with walking velocity in CP children were analyzed to provide bases for clinical treatments. Re- suits Compared with healthy children, the comfortable walking velocity and the maximum walking velocity was lower in CP children, and so was stride length, but the stride frequence showed no difference between 2 groups. In healthy children the stride frequency and stride length in- creased when walk velocity increased, but in CP children, only stride frequency increased when walk velocity increased, the stride length didn't markedly increased. Conclusions Stride length may be one of the factors of effect on walking velocity to CP children with walking disorder. Expansion of stride length could he an important way to improve CP children's walking ability.
