摘要
探讨CD44V6表达与胃癌发生发展的关系。方法:采用免疫组化染色方法,检测20例正常胃粘膜上皮、43例异型增生和85例胃癌组织CD44V6的表达情况。结果:正常胃粘膜无CD44V6表达;异型增生CD44V6的阳性表达率为30.2%(13/43),CD44V6表达阳性率与异型增生严重程度密切相关(P<0.05);胃癌组织阳性率为74.1%(63/85),其表达阳性率和染色强度均显著高于异型增生(P<0.05),CD44V6表达强度与胃癌浸润深度(P<0.05)、淋巴结转移(P<0.01)、肿瘤生长方式(P<0.05)、静脉和淋巴管侵袭(P<0.05)及远处转移(P<0.05)密切相关,且Ⅰ、Ⅱ期胃癌CD44V6染色强度和阳性率均显著低于Ⅲ、Ⅳ期(P<0.05)。结论:CD44V6上调表达与胃癌的发生发展密切相关,并发生于癌变的早期阶段,检测CD44V6表达有助于胃癌的早期诊断和转移潜能的评估。
To evaluate the relationship of CD44 splice variant V6 expression and the gastric car-cinoma tumorigenesis and progression. Methods: The expression of CD44V6 protein was detected im-munohistochemically in 148 patients surgically resected gastric mucosa. Out of the 148 cases, 20 cases were normal, 43 cases were dysplastic and 85 cases were carcinomatous, using the murine exon V6 specific monoclonal antibody. Results: The normal gastric mucosa did not reveal expression of CD44V 6, whereas the positive rate of CD44V6 immunoreactivity was 30.2%(13/43) in dysplastic mucosa. There was significantly correlation between the grade of dysplasia (P<0.05). The positive rate in can-cer was 74.1%(63/85), it was significantly higher than that of dysplasia(P<0.05). Furthermore, the strong expression of CD44V6 was correlates with depth of invasion(P<0.05), lymph node metastasis(P <0.05), growth pattern(P<0.05), venous and lymphatic invasion, distant metastasis of gastric cancer(P <0.05); Additionally, the positive rate and staining intensity of CD44V6 of the tumors within the I and Ⅱ stages was significantly lower than that of tumors within Ⅲ and Ⅳ stages(P<0.05). Conclusions: The up-regulated expression of CD44V6 may be linked with gastric carcinoma tumorigenesis and pro-gression, and may take place during early stage of gastric carcinoma.
