摘要
目的:应用核素胃排空显像观察逆行胃电刺激(RGES)对胃排空的影响,探讨胃电刺激治疗肥胖的机理,为其技术改进和进一步临床应用提供依据。方法:肥胖志愿者13例经内镜放置胃粘膜电极,于试验开始前30min及试验过程中持续给予RGES,d2研究内容同前,只是给予假性刺激。采用99Tcm-DTPA标记混合餐观察真性及假性刺激后胃半排时间。结果:受试者RGES后胃半排时间中位数为103.79min,假性胃电刺激后受试者半排时间中位数为88.66min,其中11例受试者RGES后胃半排时间长于假性胃电刺激后胃半排时间。2组(RGES组及假性胃电刺激组)胃半排时间存在显著性差异(P<0.05)。结论:RGES后受试者胃半排时间显著长于假性胃电刺激,可延缓肥胖者胃动力,延长胃半排时间。核素胃排空显像能客观定量地评价逆行胃电刺激对胃排空的影响,为其技术改进和进一步临床应用提供依据,不失为一种无创、重复性好、具有定量和符合生理特点的检查方法。
Objective:To investigate the gastric emptying changes after retrograde gastric electrical stimulation(RGES) and the mechanism of stimulation therapy,and to provide evidence of its improvement and clinical application.Methods:The electrode of RGES was attached to the gastric mucosa via endoscope in thirteen obese subjects.The investigation lasted two days.The first day RGES were performed 30min before the investigation to the last image finished.The second day pseudo-GES were instead of RGES while the process was similar to the first day.Gastric-emptying of liquid and solids labeled by 99Tcm-DTPA after RGES or pseudo-GES were measured by scintigraphic technique.Results:The median of gastric-emptying half time were 103.79 minutes after RGES and 88.66 minutes after pseudo-GES respectively.Eleven of the 13 obese subjects presented prolonged emptying by RGES.These two groups data were tested by wilconxon' method(P0.05).Conclusion:The halftime of gastric-emptying in patients who underwent RGES is longer compared to pseudo-GES.RGES can postpone gastric motility and prolong half time of gastric emptying.Radionuclide gastric-emptying imaging can evaluate the effect of RGES on gastric emptying objectively and quantitily,so that it can provide evidence of stimulation therapy's improvement and clinical application.Radionuclide gastric-emptying imaging is an invasive and well repetitive method.It can quantitate and meet the biocharacteristic.
