摘要 目的比较早期母乳喂养与配方奶喂养对早产儿的影响,探讨早产儿合理的喂养方式。方法回顾性分析2009年9~12月在我科住院的早产儿,根据其喂养方式分成2组:母乳喂养组46例和早产儿配方奶喂养组50例。比较2组早产儿院内感染、喂养不耐受的发生率,并将2组胎便排尽时间、恢复出生体重时间、达足量喂养时间以及生后14d的体重增长进行比较。结果母乳喂养组院内感染的发生率较配方奶喂养组低(分别为4.3%和18%,P<0.05);母乳喂养组喂养不耐受的发生率较配方奶喂养组低(分别为8.7%和24%,P<0.05);母乳喂养组胎便排尽的时间显著早于配方奶喂养组[分别为(73.7±10.1)h和(92.0±21.4)h,P<0.01];母乳喂养组恢复出生体重的时间较配方奶喂养组早[分别为(6.9±1.5)d和(8.0±1.9)d,P<0.05);母乳喂养组达足量喂养的时间较配方奶喂养组早[分别为(9.8±1.6)d和(11.3±2.4)d,P<0.05);母乳喂养组生后14d的体重增长优于配方奶喂养组[分别为(199±100)g和(147±75)g,P<0.05]。结论早产儿早期母乳喂养可降低院内感染和喂养不耐受的发生率,使其较早达到足量喂养,有利于早产儿早期体重增长,故应提倡早产儿早期母乳喂养。 Objective To compare the effects of early breastfeeding and formula-fed to preterm infants,and to explore a reasonable way of feeding for preterm infants.Methods A retrospective analysis was did to preterm infants who were admitted to our department from September to December in 2009.The infants were divided into two groups according to their feeding method:breastfeeding group of 46 cases and formula-fed group of 50 cases.The incidence of nosocomial infection and of feeding intolerance were compared between the two groups.The following data were also compared between the two groups:complete meconium discharge time,time to regain birth weight,time to reach full enteral feeding and weight gain 14 days after birth.Results The incidence of nosocomial infection was lower in breastfeeding group than in formula-fed group(4.3%vs 18%, P0.05).The incidence of feeding intolerance was also lower in breastfeeding group than in formula-fed group(8.7%vs 24%,P0.05).Complete meconium discharge time was significantly earlier in breastfeeding group than in formula-fed group(73.7±10.1 hr vs 92.0±21.4 hr,P0.01).Time to regain birth weight was earlier in breastfeeding group than in formula-fed group(6.9±1.5 d vs 8.0±1.9d,P0.05).Time to reach full enteral feeding was earlier in breastfeeding group than in formula-fed group(9.8±1.6 d vs 11.3±2.4 d,P0.05).Weight gain 14 days after birth in breastfeeding group was more than in formula-fed group(199±100g vs 147±75g,P0.05).Conclusion Early breastfeeding for preterm infants can reduce the incidence of nosocomial infection and feeding intolerance.It let the preterm infants to reach earlier full enteral feeding and to gain more weight in early period after birth.Early breastfeeding should be promoted in preterm infants.
