摘要
目的:探讨早期纯母乳喂养对极低出生体重儿IgA水平和感染状况的影响。方法:选取极低出生体重儿86例,随机分为观察组43例和对照组43例,观察组实行早期纯母乳喂养,对照组实行早期配方乳喂养,分别观察两组患儿出生后24 h内和7天后体内IgA水平,记录两组患儿4周内发生感染情况及使用抗生素的天数。结果:两组患儿出生后24 h内体内IgA无明显差异(P>0.05);出生7天后,观察组患儿体内IgA水平显著高于对照组患儿,也高于出生24 h内IgA水平,差异均有统计学意义(P<0.05);对照组患儿出生7天后与出生24 h内体内IgA水平差异无统计学意义(P>0.05);4周内观察组患儿发生感染的例数及使用抗生素的平均天数均显著少于对照组患儿(P<0.05)。结论:早期纯母乳喂养可以明显提高极低出生体重儿体内的IgA水平,提高患儿抵抗力,应大力提倡早期纯母乳喂养。
Objective: To explore the effect of early exclusive breastfeeding on IgA level and infection status of very low birth weight infants. Methods: Eighty- six very low birth weight infants were selected and randomly divided into observation group and control group, 43 infants in each group; the infants in observation group were fed with early exclusive breastfeeding, while the infants in control group were treated with early formula milk feeding; IgA levels within 24 hours after birth and at seven days in the two groups were observed respectively; the prevalences of infection and application days of antibiotics within four weeks in the two groups were recorded. Results: There was no statistically significant difference in IgA level within 24 hours between the two groups ( P 〉 0. 05 ) ; at seven days after birth, IgA level in observation group was statistically significantly higher than that in control group, which was also statistically significantly higher than those within 24 hours ( P 〈 0. 05 ) ; in control group, there was no statistically significant difference in IgA level between seven days after birth and within 24 hours after birth ( P 〉 0. 05 ) ; the number of infants with infection and the mean application days of antibiotics within four weeks in observation group were statistically significantly less than those in control group ( P 〈 0. 05 ) . Conclusion: Early exclusive breast- feeding can significantly improve the level of IgA in very low birth weight infants and raise resistance of them, which should be strongly advo- cated.
