多种听觉脑电生理学指标在首发精神分裂症患者诊断中的价值 被引量:2

Value of multiple auditory brainstem electrophysiology parameters in diagnosis of nave schizophrenia

摘要 目的:分析听觉脑干反应(ABR)、感觉门控电位P50(P50)、听觉诱发电位(AEP)和事件相关电位P300(P300)在辅助诊断精神分裂症中的作用。方法:对58例首发精神分裂症患者和108名正常成人应用美国Nicolet Brovo脑诱发电位仪进行P50、ABR、AEP和P300检测,并随访3个月。结果:①K-means聚类分析发现,S1-S2、S2/S1、100(1-S2/S1)、靶P3潜伏期、靶P3波幅等5个指标具有较高的灵敏度、特异度和准确率(分别为94.83%、84.26%和79.09%)。②主成分分析结果显示,对S1-S2、S2/S1比值、100(1-S2/S1)、靶P3潜伏期、靶P3波幅等5个指标得到的2个主成分,其中在第一主成分上能区分精神分裂症患者和正常成人。③一次判别分析显示,P50中的3个指标有100(1-S2/S1)、S2/S1比值和S1-P50潜伏期(S1-P50 lat)能有效地从正常成人中判别出精神分裂症患者。④二次判别分析结果显示,能够完全把判别集中的正常成人和精神分裂症患者区别出来。⑤分别应用一次判别分析和二次判别分析得到的判别方程式对随访3个月的首发精神分裂症患者进行判别,结果显示均能完全把患者准确地判别出来。结论:指标P50具有辅助诊断精神分裂症的价值,诊断价值高于指标AEP和P300。 Objective To analyze the diagnostic value of auditory brainstem response(ABR),sensory gating potential P50(P50),auditory evoked potential(AEP),and event-related potential P300(P300) in nave schizophrenia.Methods ABR,P50,AEP,and P300 were examined in 58 nave schizophrenia patients and 108 normal controls(NC) by using USA made Nicolet Brovo brain evoked potential instrument.The patients were followed for 3 months.Results ① K-means cluster analysis revealed that the five markers S2/S1 ratio,S1-S2,100(1-S2/S1) of P50,P3 latency and amplitude of P300 have high sensitivity,specificity,and accuracy rates in discriminating schizophrenia patients from NC,the sensitivity,specificity and accuracy rates were 94.83%,84.26%,and 79.09%,respectively.②Principal component analysis showed that,the first principal factor of the two principal factors derived from five markers S2/S1 ratio,S1-S2,100(1-S2/S1) of P50,P3 latency and amplitude of P300 could discriminate schizophrenia patients from NC.③ A discriminant analysis showed that three markers S2/S1 ratio,S1-P50 lat,100(1-S2/S1) of P50 could accurately discriminate schizophrenia patients from NC.④Quadratic discriminant analysis showed that they could accurately discriminate schizophrenia patients from NC.⑤Schizo phreniapatients followed three months could be accurately predicted using the discriminant formula derived from a discriminant analysis and quadratic discriminant analysis.Conclusions Sensory gating potential P50 has an auxillary value for the diagnosis of schizophrenia,the diagnostic value of sensory gating potential P50 is higher than AEP and P300.
