碘伏和苯妥因局部用药对放烧复合伤创面的治疗研究 被引量:6

摘要 目的:用大鼠放烧复合伤模型观察碘伏与本妥因(PHT)局部单用与伍用的抗感染和促愈效果。方法:复制大鼠15%Ⅱ度烧伤和4Gy60Coγ辐照复合15%Ⅲ度烧伤的损伤模型,观察不同给药方案治疗后30d存活率、创面残留率、创面感染率、肉芽组织蛋白与羟脯氨酸含量以及血管生成等指标变化。结果:碘伏单用和与PHT伍用在两种损伤均显示显著的抗感染作用和促愈作用,创面感染率、组织菌量30d创面残留率均低于对照组,而肉芽组织蛋白、羟脯氨酸含量及血管密度计数用药后7~24d两种损伤模型相继高于对照组。两药伍用未见拮抗效应,而且无论是抗感染还是促愈效果均优于单用。PHT促进蛋白、羟脯氨酸的合成代谢与促血管形成作用可能是其促愈效应的生化和形态学基础。结论:碘伏与PHT局部伍用可起抗感染和促进创面愈合的良好作用。用于放烧复会伤的治疗有重要的搭在应用价值。 ObJective: To observe the anti-infection and heal-promoting effects of topically-applied iodophor and phenytoin singly or combinedly on combined radiation-burn wouds in rats. Methods: Rats were inflicted with 15% TBSA full thickness burns or combined injury of 4 Gy gamma ray irradiation and 15% TBSA full thickness burns and then the effects of different regimens and dosages of iodophor and phenytoin on the survival rate, the percentage of residual wound surface, the wound infection rate, the protein and hydroxyproline content in the granulation tissue, and the condition of angiogenesis were observed in the rats on the 30th d after injury. Results: ①Iodophor alone or in combination with phenytoin possessed exce1lent antiinfection capacity. The wound infection rate and tissue bacterial count in iodophor-treated group were significantly lower than those in the control. ②Phenytoin alone or in combination with iodophor markedly improved wound healing, which was evidenced by the significant increase of protein and hydroxyproline in the granulation tissue, the argumentation of angiogenesis on 7th to 21st d postinjury and the decrease of the percentage of residual wound surface on 30th d. ③ When the 2 agents were administered in combination, no antagonism but agonism existed since better effects to promote healing and control infection were observed. ④The effect of phenytoin on the promotion of protein and hydroxyprolin synthesis and on angiogenesis might be the biochemical and morphological basis to augment healing. Conclusion: Topically applied iodophor and phenytoin are synergistic to promote healing and suppress infection and exert great effects on the treatment of combined radiation-burn wounds.
