摘要
目的对糖尿病并发腕管综合征患者的神经传导速度测定表现进行分析。方法使用美国牛津公司生产的OXFOR MEDELICE 5通道肌电-诱发电位仪进行检测,采用表面电极对每个患者检查对称的正中神经、尺神经、腓总神经、胫神经8条神经,共600条记录NCV。重点对正中神经进行分段检测。结果在所有患者中,发现有糖尿病周围神经病变患者有106例,腕管综合征有37例,其中有腕管综合征临床体征者有15例,其余并无明显感觉手部不适;除了腕管综合征还有其他周围神经病变者有31例,单纯发现腕管综合征者有6例,在发现的腕管综合征患者中,年龄最小30岁最大84岁,平均54岁,男性12例,女性25例,糖尿病病程由1月至20年不等,平均9年。以正中神经感觉神经传导异常居多,有29例,其中6例完全无法引出,所有患者均有腕部以下段传导异常,运动神经传导异常19例,均为腕部有下段传导异常。结论糖尿病患者中并发腕管综合征的患者并不少见,神经传导速度测定可以及早发现糖尿病并发的腕管综合征,使患者能及时得到治疗。
Objective To analyze the nerve conduction velocities in diabetes mellitus patients with carpal tunnel syndrome. Methods Examined 8 nerves: bilateral median nerves, ulnar nerves, common perone- at nerves and tibial nerves by surface electrodes using the OXFOR MEDELICE 5 channels myoeleetricity-evoked potential machine produced by OXFOR Corporation, USA. Measured the nerves conduction velocities (SCV) of median nerves, ulnar nerves, tibial nerves and superficial peroneal nerves and got 600 NCVs in all. Tile exami- nation to median nerves was of first importance and segmental tests are made. Results There were 106 patients with diabetic peripheral neuropathy, 37 patients with carpal tunnel syndrome among whom clinical signs were found in 15 patients and the rest had no discomfort in their hands, 31 patients with other peripheral neuropathy besides carpal tunnel syndrome, and 6 patients with carpal tunnel syndrome only. 12 males and 25 females were included in the patients that were diagnosed to have carpal tunnel syndrome and the ages were between 30 to 84 years, 54 years in average. The courses of diabetes mellitus ranged from 1 month to 20 years, 9 years in average. Abnormality of median nerves sesory conduction were detected in 29 patients containing 6 with no elic- itation. Abnormal sensory conductions of the parts distal to wrists were confirmed in all these patients. Abnor- mal motor conductions of the parts distal to wrists were revealed in 19 patients. Conclusion The phenomenon that diabetes mellitus complicating with carpal tunnel syndrome was quite common. Detecting nerves conduction velocity was helpful to find out carpal tunnel syndrome in diabetes mellitus so treatment could be made in time.
