人体肌纤维传导速度测定

摘要 目的:探讨肱二头肌的肌纤维传导速度(musclefiberconductionvelocity,MFCV)测定对于肌源性或神经源性肌肉损害的判断。方法:实验于1987-04/2000-03在重庆医科大学附属第一医院神经内科完成。参照Chino1984年介绍的方法,对98例皮肌炎患者和59例多发性肌炎患者、进行性肌营养不良症患者进行了肌纤维传导速度测定。结果:发现此类患者在出现相应临床症状时,MFCV均较正常人减低,测得此两类患者MFCV为(3.59±0.57),(3.50±1.66)m/s。经治疗后临床症状逐渐消失,MFCV则逐渐至正常。为了进一步探讨MFCV对肌肉疾病的诊断和预后价值,以肌病组与正常人组、失神经损害疾病组、一般神经系统疾病组(即头痛、失眠等)对照MFCV测定并进行了肌皮神经潜速率测定以除外神经损害因素。结论:MFCV可作为协助肌源性疾病诊断的一项指标,且可将此检查作为治疗效果的动态观察。 AIM:To probe into the muscle fiber conduction velocity(MFCV),and estimat the judgment of myogenic and neurogenic intramuscular injury.METHODS:The experiment was finished in the Department of Neurology,First Affiliated Hospital of Chongqing Medical University from April 1987 to March 2000.Ninety- eight patients with dermatomyositis and 59 patients with polymyositis and progressive muscular dystrophy(PMD) were estimated in MFCV according to the methods introduced in Chino in 1984.RESULTS:When the corresponding clinical symptoms occurred,MFCV in the patients was lower than that in normal persons.The results indicated that MFCV in the two kinds of patients was(3.59± 0.57) m/s and(3.50± 1.66) m/s.The clinical symptoms disappeared after treatment and MFCV resumed to normal level.MFCV in myopathy group was compared with that in normal person group,denervation group,normal nervous system disease(headache, agrypnia,etc.) group,and the motor nerve conduction velocity in musculocutaneous nerve was detected to exclude the factors of nerve damage,in order to study the diagnosis and value of MFCV in myopathy.CONCLUSION:MFCV can be used as one of the assistant indexes to diagnose myogenic disease.Furthermore,the determination can be considered as dynamic observation of therapeutic efficacy.
