BiPAP无创正压通气治疗急性左心衰竭32例疗效评价 被引量:5

摘要 目的观察BiPAP无创正压通气在急性左心衰竭抢救中的临床应用疗效。方法 62例住院急性左心衰竭患者随机分为2组,治疗组32例,采用常规药物治疗加BiPAP面罩ST-D-20呼吸机双向正压通气,以流量触发,压力支持,所给吸气压8~15 cmH2O,呼气压2~5 cmH2O。对照组30例,采用常规药物治疗加鼻导管吸氧。均记录治疗前后症状、体征及血气分析各参数。结果治疗组经无创Bi-PAP面罩通气后,所有患者在10 min~2 h内呼吸频率减慢,心率下降,PaO2上升,PaCO2下降,对血压无明显影响。治疗组治疗后总有效率为93.75%,显效率为81.25%,对照组总有效率为66.67%,显效率为50.00%,两组间总有效率和显效率比较,差异有统计学意义(P<0.01)。结论 BiPAP无创正压通气治疗急性左心衰竭效果显著,迅速改善患者的症状和低氧血症,是一种快速安全有效的左心衰抢救措施。
