摘要
目的探讨无创机械通气对改善急性左心衰竭患者心输出量的作用。方法选择2015年3月至2016年10月我院收治的50例急性左心衰竭患者作为研究对象,随机分为对照组和观察组各25例,在一般药物治疗的基础上,对照组患者采用面罩吸氧,观察组患者给予无创机械通气进行干预。治疗1h后观察比较两组患者心率(HR)、呼吸频率(RR)、血氧饱和度(SPO_2)、心输出量(CO)等参数的变化。结果与治疗前相比,对照组患者HR、RR、CO、SPO_2均有所改善,但差异无统计学意义(P>0.05);观察组患者HR、RR下降,SPO_2、CO有所回升,差异有统计学意义(P<0.05);治疗1h后,观察组患者HR、RR均比对照组患者低,SPO_2、CO均比对照组患者高,组间差异有统计学意义(P<0.05);对照组总有效率为84.00%,观察组总有效率为92.00%,观察组总有效率明显高于对照组,差异有统计学意义(P<0.05)。结论应用无创机械通气配合常规治疗对于急性左心衰患者心功能及心输出量的改善具有显著疗效,具有快速缓解患者症状、改善氧供状态的作用,效果确切。
Objective To investigate the effect of noninvasive mechanical ventilation on cardiac output in patients with acute left heart failure.Methods A total of 50 patients with acute left heart failure in our hospital from March 2015 to October 2016 were randomly divided into control group(n=25)and observation group(n=25).On the basis of general drug therapy,the patients in control group were treated with mask oxygen and the observation group was given noninvasive mechanical ventilation.The changes of heart rate(HR),respiratory rate(RR),oxygen saturation(SPO2),cardiac output(CO)and other parameters of two groups were observed and compared after treatment of 1hour.Results Compared with before treatment,HR,RR,CO,SPO2 of patients in control group were improved,but the difference was not significant(P〉0.05);HR,RR of the patients in observation group were decreased,and SPO2,CO were increased,the difference was statistically significant(P〈0.05);After the treatment of 1hour,HR,RR of patients in observation group were lower than control group,and SPO2,CO were higher than control group,there was significant difference between the groups(P〈0.05);The total effective rate of observation group(92%)was significantly higher than control group(84%),there was statistical significance difference(P〈0.05).Conclusion The application of noninvasive mechanical ventilation combined with conventional therapy for improving cardiac function in patients with acute left heart failure and cardiac output volume had significant effect.It has a rapid effect on relieving symptoms and improving oxygen supply.
