原发性肾小球肾炎患者尿中结缔组织生长因子的检测及其临床意义 被引量:5

Urinary Excretion Rate of Connective Tissue Growth Factor in Primary Glomerulonephritis and Its Clinical Significance

摘要 目的探讨原发性肾小球肾炎患者尿中结缔组织生长因子(CTGF)水平的变化及临床意义。方法选择原发性肾小球肾炎患者48例,正常对照组32例,应用双抗体夹心酶联免疫吸附法(ELISA)测定其尿中CTGF分泌水平,应用比浊法检测尿蛋白定量,全自动生化分析仪检测血浆清蛋白、血肌酐、血尿素氮等生化指标,选用肾小球指数、肾小管间质病变程度作为评价肾脏病理损害的指标,并与尿中CTGF水平进行相关性分析。结果原发性肾小球肾炎组患者尿CTGF水平较对照组升高,差异有统计学意义(P<0.05)。原发性肾小球肾炎组患者尿CTGF水平与尿蛋白指数、血肌酐水平、血尿素氮水平、肾小球指数、肾间质病变程度呈正相关(r值分别为0.453、0.368、0.339、0.412、0.397,P<0.05),而与血浆清蛋白水平无明显相关性。结论原发性肾小球肾炎患者尿中CTGF水平升高,检测尿中CTGF水平可能成为评价慢性肾小球肾炎患者肾脏病变严重程度的指标。 Objective To investigate the level of connective tissue growth factor (CTGF) in the urine of patients with primary glomerulonephritis and its clinical significance. Methods Totally 48 patients with primary glomerulonephritis in our hospital and 32 normal controls were selected. ELISA method was used to assay the level of CTGF in the urine. The level of urinary protein was detected by turbidimetry, while Plasma albumin, serum creatinine, and blood urea nitrogen (BUN) were measured by auto - biochemistry analyzer. Pathological changes of kidney were evaluated by glomerular index and the degree of tubulointer- stitial fibrosis, and their correlation with CTGF were also analyzed. Results The level of CTGF in the urine of patients with pri- mary glomerulonephritis was increased ( P 〈 0. 05 ), which was in positive correlation with priteinuria index, serum creatinine, BUN, glomerular index and the degree of tubulointerstitial fibrosis (r=0.453, 0.368, 0.339, 0.412, 0.397, respectively, P 〈 0. 05 ), but not significantly correlated with the level of plasma albumin. Conclusion The level of CTGF in the urine of pa- tients with primary glomerulonephritis was increased. Detection of urinary CTGF may be an indicator for evaluating the severity of kidney disease in the primary glomerulonephritis.
