摘要
目的:为了探讨肝硬化和消化性溃疡患者血管活性肠肽(VIP)的变化及其调节机制。方法:测定了20例肝硬化和20例消化性溃疡患者的血浆VIP水平。结果:肝硬化[(140.25±28.23)ng/L]和消化性溃疡[(118.50±26.26)ng/L]患者的血浆VIP水平均昆著高于20例健康对照组[(1030±19.56)ng/L],P分别<0.001和<0.05。肝硬化并发食管静脉曲张者显著高于无食管静脉曲张者[(153.18±23.69)ng/L与(124.44±26.15)ng/L,P<0.02],肝硬化有腹水者也高于无腹水者[(158.88±22.23)ng/L与(129.17±27.12)ng/L,P<0.05]。肝硬化患者血浆VIP浓度与凝血酶原时间呈正相关(r=0.68、P<0.01),与血清白蛋白浓度呈负相关(r=-0.59,P<0.01)。胃溃疡和十二指肠溃疡患者血浆VIP水平无显著差异[(114.50±24.55)ng/L与(122.50±28.60)ng/L,P>0.05]。结论:这些患者的血浆VIP水平升高可能与VIP分泌增加、肝脏灭活减少、清除降低和(或)存在门体分流有关。
Objective:To investigate the changes of vasoactive intestinal peptide(VIP) in patients with hepatic cirrhosis and peptic ulcer and its regulating mechanism. Methods:Plasma VIP levels in 20 patients with hepatic cirrhosis and 20 with peptic ulcer were measured.Rcsult:The levels of VIP in patients with hepatic cirrhosis (140.25 ± 28.23)ng / L and peptic ulcer (118.50±26.26)ng / L were significantly higher than those in 20 healthy control subjects (103.0 ± 19.56)ng / L, P < 0.001 and P < 0.05,respectively. The levels of VIP in caesc of hepatic cirrhosis complicated with esophageal varices were significantly higher than those without esophageal varices (153. 18 ± 23.69)ng / L vs;(124.44 ± 26. 15)ng / L, P < 0.02) and the levels of VIP in patients associated with ascites were also higher than those without (158.88 ± 22.23)ng / L vs, (129. 17 ± 27. 12)ng / L, P < 0.05. Plasma VIP levels in patients with hepatic cirrhosis were positively correlated with prothrombin time (r= 0.68,P < 0.01), and negatively correlated with albnmin (r= -0.59, P < 0.01). There was no difference of plasma VIP levels between the patients with gastric ulcer and those with duodenal ulcer (114.50 ± 24.55)ng / L vs.(122.50 ± 28.60)ng / L, P > 0.05).Conclusion:Incrcased secretion of VIP, impaired inactivation of VIP in the cirrhotic liver, decreased clearance of VIP and / or the presence of portosystemic shunt may contribute to the high plasma levels of VIP in these patients.
