The treatment of the super senilities with intertrochanteric fractures

摘要 目的:观察经皮加压钢板内固定术(PCCP)与经皮空心钉内固定术对不同类型高龄股骨转子间骨折的疗效。方法:收集5例行经皮加压钢板内固定术与8例行空心钉内固定术高龄股骨转子间骨折患者的临床资料,观察两种手术方式的手术持续时间,出血量以及术后卧床时间。使用日本整形外科协会JOA髋关节判定评分标准和Barthel指数评分法对两种手术方式患者进行术前、术后1个月、6个月、12个月评分。结果:统计学分析表明,两种手术方式在手术持续时间以及手术出血量方面无统计学差异,用PCCP治疗的患者卧床时间明显短于空心钉治疗的患者。结论:对有PCCP适应证的高龄股骨转子间骨折患者应尽早行此手术治疗,以减少卧床时间,减少卧床并发症的发生率。 Objective: To compare percutaneous compression plating (PCCP) internal fixation with percutaneous cannulated screws internal fixation in the treatment of the senilities with intertrochanteric fractures. Methods: 13 correlated hospitalized patients were selected to compare surgical enduration time, bleeding volume and the bedridden time after surgery. By the Japanese Orthopaedic Association scoring system (JOA hip joint score)and Barthel index scoring system, the patients were scored for preoperation,one month,six months and twelve months after operation. Results: Surgical enduration time, bleeding volume were expressed no statistical differences in two groups,but the bedridden time was shorter in patients with PCCP internal fixation than in patients with percutaneous cannulated screws internal fixation. Conclusion: The senilities with intertrochanteric fractures who are suitable for PCCP should be operated with it in the early time. In this way, the patients can lessen the time of the bedridden and incident rate of complication.
