乳腺癌保留乳房治疗复发相关危险因子研究 被引量:10

Analysis of relative risk factors of local recurrence after conservative operation in breast cancer women

摘要 目的:研究乳腺癌保留乳房手术术后复发的相关危险因素。方法:回顾分析我院2000-01-2005-12行乳腺癌保留乳房手术289例,将其中术后局部复发的35例与无复发病例进行临床与病理对比分析,寻找临床病理资料相关危险因子。结果:复发组与未复发组相比,患者的年龄较小(P<0.01),肿瘤体积较大(P<0.01),肿瘤距乳头距离较近(P<0.01),ER及PR阴性率较高(P<0.01),Her-2过表达率较高(P<0.01),有乳腺癌家族史比例较高,P<0.01。而手术方式、病理类型、淋巴结数目、组织学分级、术后化疗与放疗等方面则差异无统计学意义。结论:对于乳腺癌保留乳房手术,应该个体化综合评价可能引起复发的各项危险因素,选择适当与规范的治疗方案,从而获得满意的治疗效果。 OBJECTIVE:To explore the relative risk factors of local recurrence after breast cancer conservative operation. METHODS:There were 289 cases of breast cancer conservative operation from 2000-01-2005-12,and 35 cases occurred local recurrence among it. The clinical and pathological analysis was performed in these cases to find the relative risk factors. RESULTS:Between non-recurrence group and recurrence group,there were statistical by significant difference in the patient's age(P0.01),tumor size(P0.01),distance between tumor and nipple(P0.01),status of hormonal receptor(P0.01),and family history of breast cancer(P0.01),but there were no statistically significant difference in pathological type,lymph node number,histological grade,chemotherapy and radiotherapy regimen. CONCLUSION:For the breast cancer conservative operation,the various risk factors should be assessed,which would cause local recurrence individually and synthetically,and should be choosed a prompt and standard treatment regimen to achieve satisfying effect.
