摘要
目的探讨采用巩膜外冷凝垫压术治疗陈旧性视网膜脱离的临床效果。方法采用回顾性、非对照研究对2005年6月至2008年6月于我院眼科确诊为陈旧性视网膜脱离的53例(60眼)患者行巩膜外冷凝垫压术治疗,其中采用单纯巩膜外垫压术20眼(33.3%);采用巩膜外垫压环扎术40眼(66.7%);术中联合消毒空气玻璃体内注射35眼(58.3%);部分患者给予视网膜激光光凝术以促使视网膜复位。并对视网膜复位情况、视力预后及其与发病时间的关系等进行分析。结果所有患者术后随访1a以上,平均14个月。末次随访时,44眼视网膜完全复位治愈;11眼局部视网膜下少量积液,未波及黄斑,稳定无进展;5眼在施行玻璃体注气术后仍然无法使视网膜贴附。一次手术治疗陈旧性视网膜脱离的有效率为91.7%(55眼),治愈率为73.3%(44眼)。巩膜外冷凝垫压术治疗陈旧性视网膜脱离可以显著提高患眼的视力(χ2=38.200,P=0.000)。结论巩膜外冷凝垫压术以其简便和可重复性不失为治疗陈旧性视网膜脱离的一种简单、有效的手段。
Objective To investigate the curative effects of scleral condensing and buckling operation on long-standing retinal detachment.Methods Retrospective and non-comparative study was used,53 patients(60 eyes) with long-standing retinal detachment from June,2005 to June,2008 were treated with single scleral buckling operation in 20 eyes(33.3%),scleral buckling and cerclage operation in 40 eyes(66.7%),combined with disinfectant air vitreous injection in operation in 35 eyes(58.3%),and retinal photocoagulation was performed in some patients to promote the retinal reattachment.Retinal reattachment state and relationship between prognosis of visual acuity and duration of the disease were analyzed.Results The follow-up duration was more than 1 year(average 14 months).At the last follow-up,retina was completely reattached in 44 eyes,partially reattached and little sub-retinal fluid localized with no involving macular and progression in 11 eyes,remained detached after disinfectant air vitreous injection in 5 eyes.The effective rate of treating long-standing retinal detachment once a time was 91.7%(55 eyes),the healing rate was 73.3%(44 eyes) mainly based on the examination of ocular fundus.The scleral condensing and buckling for long-standing retinal detachment obviously improved the visual acuity(χ^238.200,P=0.000).Conclusion Scleral condensing and buckling operation is an easy and effective measure in treating the long-standing retinal detachment.
