Effect of changes of serum estradiol concentration in luteal phase of IVF-ET on embryo implantation

摘要 目的:探讨超促排卵周期胚胎移植日血清雌二醇(E2)浓度的变化对体外受精-胚胎移植(IVF-ET)治疗结局的影响,为制定个体化黄体支持方案提供参考。方法:回顾性分析125个IVF-ET周期,按ET日血清E2浓度变化分为4组:A组:24个治疗周期,ET日血清E2浓度/HCG注射日E2浓度<0.4;B组:54个周期,0.4≤ET日E2/HCG日E2<0.7;C组:32个周期,0.7≤ET日E2/HCG日E2<1;D组:15个周期,ET日E2/HCG日E2≥1。结果:C组胚胎种植率和妊娠率最高,A组种植率和妊娠率均显著降低(P<0.05)。结论:IVF-ET采用长方案控制性超排卵(COH),移植日E2水平过高或过低均影响治疗结局,监测E浓度对制定个体化黄体支持方案具有重要的临床价值。 Objective: To explore the effect of changes of serum estradiol concentration on the day of human ehorionie gonadotropin (HCG) administration in luteal phase on the outcomes of in vitro fertilization - embryo transfer ( IVF - ET), provide reference for making individual corpus lutealum support scheme. Methods: 125 IVF -ET cycles were analyzed retrospectively, then divided into 4 groups according to changes of serum estradiol concentration on the day of HCG administration : group A : 24 cycles, the ratio of estradiol concentration on ET day/estradiol concentration on HCG day 〈 0. 4 ; group B : 54 cycles, 0. 4 ≤ the ratio of estradiol concentration on ET day/estradiol concentration on HCG day 〈 0. 7 ; group C : 32 cycles, 0. 7 ≤ the ratio of estradiol concentration on ET day/estradiol concentration on HCG day 〈 1 ; group D: 15 cycles, the ratio of estradiol concentration on ET day/estradiol concentration on HCG day≥1. Resets: The implantation rate and pregnancy rate in group C were the highest, and the implantation rate and pregnancy rate in group A decreased significantly ( P 〈 0. 05) . Conclusion: For the cases adopting long protocol of controlled ovarian hyperstimulation (COH) in IVF - ET, too high or two low estradiol concentration on ET day may affect the outcomes, monitoring estradiol concentration is of clinical value in making individual corpus luteum support scheme.
