辐照猪皮与重组人表皮生长因子凝胶治疗小面积Ⅱ度烧伤的临床观察 被引量:14

Irradiated pigskin graft dressing and recombinant human epidermal growth factor gel application in treatment of small-area partial-thickness burn:report of 135 cases

摘要 目的观察辐照猪皮与重组人表皮生长因子(recombinant human epidermal growth factor,rhEGF)凝胶用于治疗Ⅱ度烧伤的临床疗效。方法选择135例Ⅱ度烧伤患者,分成3组,分别是辐照猪皮组、rhEGF凝胶组和对照组(碘伏油纱组),观察创面愈合时间、患者疼痛度、换药工作量、色素沉着度、愈合后效果以及1年后随访效果。结果不同治疗方式的愈合时间:对照组>rhEGF凝胶组>辐照猪皮组。辐照猪皮组和rhEGF凝胶组对Ⅱ度烧伤创面的治疗效果均优于对照组,平均愈合时间提前3~5 d,差异有统计学意义(P<0.01),且辐照猪皮组效果要略优于rhEGF凝胶组(P<0.05);rhEGF凝胶组和辐照猪皮组的患者疼痛度减轻及换药工作量相对对照组明显减少(P<0.01),且患者创面愈合效果良好、较少引起色素沉着,1年后部分患者随访证实远期愈合效果好,较少引起瘢痕增生。结论辐照猪皮和rhEGF治疗Ⅱ度烧伤效果确切、预后良好,临床上值得广泛使用。 Objective To investigate the therapeutic effect of the application of recombinant human epidermal growth factor (rhEGF) gel and the dressing of the irradiated pigskin graft in the treatment of small-area partial-thickness burn injury, and compare the outcomes with topically use of povidone iodine. Methods Totally 135 eases of patients with TBSA 〈 10% partial-thickness burns admitted in our hospital from March 2007 to March 2008 were randomly divided into 3 groups: rhEGF gel groups (n = 53) , irradiated pigskin graft group (n =40) and PVP-I groups (povidone iodine, n = 53). The wound healing time, prognosis and results after 1 year' s follow-up. Results With different treatments, the healing time : PVP-I group 〉 rhEGF gel group 〉 irradiated pigskin graft group [ (14.42 ± 2.10)d vs (9.82 ± 1.52 )d vs (8.09 ± 1.05 )d 1. Irradiated pigskin group and rhEGF gel group had better effect than the control group in the treatment of partial-thickness burn patients, and significantly shorten the average healing time of 3 to 5 d ( P 〈 0. 01 ). Irradiated pigskin graft group was slightly better than the rhEGF gel group ( P 〈 0. 05 ), but the difference was not as significant as with the control group. Irradiated pigskin graft group and rhEGF gel group had good healing effect: lesser pain and doctor' s workload (P 〈 0. 01 ) and lesser pigmentation and fewer scars in some patients after 1 year' s follow-up. Conclusion Irradiated pigskin graft and rhEGF have exact effect and sound prognosis in the treatment of partial-thickness burn, and they should be widely used.
