摘要
目的筛选能反映非小细胞肺癌增殖、凋亡和细胞周期进程速度的CT征象和临床病理指标。方法应用免疫组织化学sP法,检测33例(鳞癌11例,腺癌22例)术前行18F—FDGPET/CT扫描的肺癌组织的Ki-67、Survivin及CyclinA的表达情况。分析肺癌的CT征象、病理类型、分化程度、有无纵隔淋巴结转移与上述免疫组织化学指标的关系。结果Ki-67、Survivin和CyclinA的阳性表达率分别为69,70%、84.85%和45.45%;鳞癌的Ki-67和CyclinA的阳性表达率显著高于腺癌(P=0.031和P=0.016);Ki-67、Survivin和CyclinA的阳性表达率与分化程度无关(P〉0.05);≥2cm组Ki-67阳性表达率显著高于〈2cm组(P=0.025);有胸膜凹陷征组CyclinA阳性表达率显著高于无胸膜凹陷征组(P=0.032);无血管集束征组CyclinA阳性表达率显著高于有血管集束征组(P=0.027)。结论肺鳞癌处于增殖阶段的细胞数目和细胞周期进程速度高于腺癌;大小可反映肿瘤细胞的增殖能力;有胸膜凹陷征者肿瘤细胞周期的进程速度较快。
Objective To select the CT characteristics and clinicalopathological parameters of non - small lung cancer which can reflect the expression of Ki - 67,survivin and cyelinA protein. Method 33 cases of lung cancer(squamous carcinoma 11 cases and adenocarcinoma 22 eases)have undergone the scanning of 18F - FDG PET/CT before operation. The relationships between CT features, pathological classification, tumor differentiated types,lymph node metastasis and the expression of Ki - 67, survivin and eyclinA which were examined by means of SP immunohistochemical technique. Results The positive rates of Ki - 67 ,survivin and eyelinA protein were 69.70% ,84.85% and 45.45% respectively. Significant differences in the positive rate of led - 67 and cyclinA were found between squamous carcinoma and adenocareinoma (P = 0. 031 and P = 0. 032 )and between the size ≥2 cm and 〈2 cm(P =0.025) ,but not among differentiated types,lymph node metastasis and CT feature of lobulation,thin and short spiculation, spinous protuberance,cavity sign, vessel convergence sign and pleural retraction sign(P 〉0.05). The positive rate of eyclinA was also related to vessel convergence sign and pleural retraction sign ( P = 0. 027, P = 0. 032 ), but not related to other data ( P 〉 0.05 ). There were no correlations between the positive rate of survivin and other data. Conclusion The study suggested that the ability of proliferation and the speed of cell cycle of squamous carcinoma were stronger than that of adenocareinoma. The size of tumor can reflect the stronger ability of proliferation. The pleural retraction sign can reflect the faster cell cycles.
