摘要
【目的】观察不同胎龄早产儿脑干听觉诱发电位(brainstem auditory evoked potential,BAEP)的特点,并与足月儿比较,探讨其发育规律。【方法】排除听力及脑损伤危险因素的早产儿68例为研究组,同期住院分娩的无听力及脑损伤危险因素的足月新生儿为对照组,早产儿于生后3~7d及矫正胎龄37~42周,足月儿生后3~7d分别进行BAEP检测,比较各组BAEP各波形参数。【结果】80dBnHL短声刺激下,早产儿和足月儿BAEP各波形中均以Ⅰ波、Ⅲ波和Ⅴ波分化发育较清晰;早产儿随胎龄增加Ⅰ、Ⅲ、Ⅴ波潜伏期及峰间期逐渐缩短,波幅则呈增高趋势,组间差异有显著性(P〈0.01);早产儿组在矫正胎龄时的BAEP波形与足月儿组比较,Ⅰ、Ⅲ、Ⅴ波潜伏期,峰间期及各波振幅差异均无显著性(P〉0.05)。【结论】早产儿的脑干诱发电位发育相对不成熟,听觉神经通路存在一个不断完善的过程,对早产儿BAEP的分析不能套用足月儿的判断参数。
[Objective] To investigate the developmental characteristics of brainstem auditory evoked potential in preterm infants and compare it to term neonates so that to explore developmental rule brainstem auditory evoked potential in preterm infants. [Methods] Preterm infants without the factors on hearing and brain damage were selected into studygroup (n=68) and the term neonates deliveried in hospital at the same time were as control group. Brainstem auditory evoked potential was performed at 3 to 7days after birth and 37 to 42 weeks corrected gestational age in preterm infants, while only at 3 to 7days after birth in the term neonates. The characteristics and testing parameters of brainstem auditory evoked potential were summarized. [Results] With a click intensity of 80 dBnHL, wave Ⅰ、Ⅲ、Ⅴ of brainstem auditory evoked potential in two groups were clear. The peak lateneies of wave Ⅰ、Ⅲ、Ⅴand peak intervals Ⅰ-Ⅲ、Ⅲ-Ⅴ, Ⅰ-Ⅴ were shortening and which amplitude had the tendency with gestational age. The differences in groups were significant (P 〈0.01). No significant differences were showed between the term neonates and the preterm infants 37 to 42 weeks correc ted gestational age (P〉0.05). [Conclusions] Development of hrainstem auditory evoked potential in preterm infants is immature and it is a continuous process. The parameter standard of the term neonates is not suitable for preterm infants.
