急性白血病多药耐药基因检测及其临床意义

DETECTION OF MULTIDRUG RESISTANCE GENE 1 EXPRESSION IN ACUTE LEUKEMIA AND ITS CLINICAL IMPLICATION

摘要 应用原位杂交方法检测多药耐药基因(Mdrl)表达,观察其与临床耐药的关系,探讨Mdrl检测的临床应用价值。方法自行构建RNA探针直接在骨髓涂片上行原位杂交,检测45例急性白血病MdrlmRNA表达。对部分高表达者调整临床治疗方案。结果复治组中Mdrl阳性占88%,初治组42.8%,有极显著差异。经标准化疗2个疗程后初治组中Mdrl阳性者完全缓解率(CR)22%,阴性者CR63.6%,P<0.001。对Mdrl阳性病例通过调整化疗方案或加用逆转剂可改善预后。结论Mdrl高表达确为白血病不良预后的主要指标,原位杂交检测Mdrl表达对临床治疗有指导意义。 PURPOSE To observe the relation between multidrug resistance gene1 (Mdr1) expression and clinical drug resistance and to evaluate the clinical value of the Cytospin in situ hybridization(ISH) technique preliminarily. METHODS ISH for Mdr1 mRNA was performed with Dig-nick translated RNA probes and anti-Dig alkaline phosphatase detection system in 45 patients with acute leukemia. The chemotherapy regimens of a few cases with high Mdr1 expression were then regulated. RESULTS The positive rate of Mdr1 expression in relapse and refractory patients was much more higher than that in initial cases ( P <0.001). Complete remission rate was obviously lower in overexpression cases of the initial acute leukemia group. Better prognosis was obtained through modulation of remission induction regimens or addition of reversal agents. CONCLUSIONS Overexpression of Mdr1 mRNA is truly one of the important indication of poor prognosis in acute leukemia and the ISH method can be used satisfactorily in predicating clinical drug-resistance and directing the rearrangement of chemotherapy regimens.
