摘要
脑室内出血常发生在孕龄小于32周的早产儿.许多学者认为与毛细血管的末成熟有关.该文电镜观察了两例早产儿侧脑室脉络丛和脑室壁的标本.用抗IV型胶原和laminin抗体标记10例死于脑室内出血的早产儿和10例死于非脑室内出血的早产儿脉络丛和室壁的标本.见到毛细血管无完整的基膜,既有不连续的基膜样物质.IV型胶原和laminin阳性产物呈不连续线状.脑室内出血组的IV型胶原和laminin阳性产物的平均面积分别比非脑室内出血组的小,t检验P<0.05.侧脑室脉络丛和脑室壁毛细血管基膜发育落后于脑实质的基它部位,既有不连续的基模样物质和较少的IV型胶原及laminin,是侧脑室毛细血管未成熟的形态学特征之一,和脑室内出血易发生的早产儿的原因之一.
Intraventricular hemorrhage (IVH), which is generally believed to be relate to premature capillaries of ventricles, occurs easily in neonates of less than 32 gestational weeks. Choroid plexuses and the walls of the lateral ventricles in two prematures were observed by electron trans-mission microscopy. Anti-collagen IV and anti-laminin antibodies of choroid plexuses and the walls of lateral ventricles were labeled in 10 prematures who died of IVH and in 10 prematures who died of non-IVH. No intact basement membranes of capillaries were found in choroid plexus-es and the walls of the lateral ventricles, but only discontinuous, cloudy basement-membrane-like materials. Positive products of collagen IV and laminin presented discontinuous linear. The average area of the IVH group was smaller than that of the non-IVH group(P<0. 05). The de-velopment of capillary basement membrane in the choroid plexus and the walls of the lateral ven-tricle lagged behind that of other parts of the brain in prematures. There were basement-mem-brane-like material and less collagen IV and laminin. These are morphological features of prema-ture capillaries and the causes for IVH in prematures.
