妊娠期高血压疾病患者血清及胎盘中AngⅡ及胎盘中AT_1R水平变化的研究 被引量:1

Investigation on changes of angiotensin Ⅱ level in serum and placenta and angiotensin Ⅱ type Ⅰ receptor level in placenta of patients with hypertensive disorder complicating pregnancy

摘要 目的:检测妊娠期高血压疾病患者母血及胎盘组织中血管紧张素Ⅱ(AngⅡ)及胎盘组织中血管紧张素Ⅱ的Ⅰ型受体(AT1R)水平的变化,以探讨AngⅡ及AT1R在妊娠期高血压疾病发病中的作用。方法:采用酶联免疫吸附双抗体夹心法(ELISA)检测60例妊娠期高血压疾病患者及20例正常妊娠妇女血清中AngⅡ水平,同时采用免疫组织化学方法检测胎盘组织中AngⅡ及AT1R的表达。结果:妊娠期高血压疾病组血清中AngⅡ水平明显高于正常妊娠组(P<0.01)。重度子痫前期组较轻度子痫前期组血清AngⅡ水平明显升高(P<0.01),轻度子痫前期组高于妊娠期高血压组(P<0.05)。妊娠期高血压疾病组与正常妊娠组比较胎盘组织中AngⅡ表达明显升高(P<0.01)。妊娠期高血压组、轻、重度子痫前期组胎盘组织中AngⅡ表达逐渐增强,各组间差异具有统计学意义(P<0.05)。妊娠期高血压疾病组胎盘AT1R表达水平显著高于正常妊娠组(P<0.01)。轻、重度子痫前期患者胎盘组织中AT1R表达强度明显高于正常妊娠组(P<0.05,P<0.01)。妊娠期高血压疾病组及正常妊娠组血清AngⅡ水平与胎盘组织AngⅡ表达均呈正相关(r=0.378,P<0.01;r=0.665,P<0.01)。AngⅡ的表达与胎盘重量及新生儿体重呈负相关(r=-0.244,P<0.05;r=-0.347,P<0.01)。结论:妊娠期高血压疾病患者血清AngⅡ水平显著升高,且随病情加重,血清AngⅡ有进一步上升的趋势,说明血清AngⅡ从一定程度上反映了病情的轻重,有望成为一种预测妊娠期高血压疾病的监测指标。妊娠期高血压疾病患者胎盘中AngⅡ、AT1R表达水平的变化与妊娠期高血压疾病的发病及病情发展有关。 Objective:To detect the changes of angiotensin Ⅱ level in serum and placenta and angiotensin Ⅱ type Ⅰ receptor(AT1R) level in placenta of patients with hypertensive disorder complicating pregnancy(HDCP),explore effects of angiotensin Ⅱ and AT1R on the pathogenesis of HDCP.Methods:60 patients with HDCP and 20 healthy pregnant women(control group) were included in the study,the level of angiotensin Ⅱ in serum was detected by enzyme linked immunosorbent assay(ELISA),and the levels of angiotensin Ⅱ and AT1R in placenta were detected by immunohistochemistry.Results:The level of angiotensin Ⅱ in serum of HDCP group was significantly higher than that of control group(P0.01).The level of angiotensin Ⅱ in serum of severe preeclampsia group was significantly higher than that of mild preeclampsia group(P0.01),and the level of angiotensin Ⅱ in serum of mild preeclampsia group was significantly higher than that of HDCP group(P0.05).The level of angiotensin Ⅱ in placenta of HDCP group was significantly higher than that of control group(P0.01).There was significant difference in angiotensin Ⅱ level in placenta among HDCP group,mild preeclampsia group and severe preeclampsia group(P0.05).The level of AT1R of HDCP group was significantly higher than that of control group(P0.01).The levels of AT1R of mild preeclampsia group and severe preeclampsia group were significantly higher than that of control group(P0.05,P0.01).There were positive correlations in angiotensin Ⅱ level of HDCP group and control group between serum and placenta(γ=0.378,P0.01;γ=0.665,P0.01).There were negative correlations between angiotensin Ⅱ expression and weight of placenta,neonatal body weight(γ=-0.244,P0.05;γ=-0.347,P0.01).Conclusion:High level of angiotensin Ⅱ in patients with HDCP reflects the degree of HDCP and can be used as a clinical monitoring target,which increases with the degree of HDCP.The changes of angiotensin Ⅱ and AT1R levels are related to the occurrence and development of HDCP.
