摘要 目的:探讨尖锐湿疣和鲍恩样丘疹病患者皮损内天然免疫和获得性免疫功能变化情况。方法:应用EnVision法对尖锐湿疣和鲍恩样丘疹病皮损进行Toll样受体(TLR)2、4及CD3、CD4、CD8、CD68免疫组化标记。结果:①尖锐湿疣和鲍恩样丘疹病皮损中均有TLR 2和4阳性表达,TLR 2在棘细胞、基底细胞、真皮浅层炎性细胞中强表达;②尖锐湿疣和鲍恩样丘疹病真皮浅中层均有以CD3^+、CD8^+、CD68^+细胞为主的明显炎性细胞浸润,尖锐湿疣浸润细胞量明显多于鲍恩样丘疹病。结论:①尖锐湿疣和鲍恩样丘疹病皮损内表皮细胞及单核巨噬细胞Toll样受体阳性表达,说明当人乳头瘤病毒(HPV)病毒入侵时,表皮细胞和单核巨噬细胞能对HPV抗原进行识别和处理。②尖锐湿疣和鲍恩样丘疹病的真皮浅中层均发生了以淋巴细胞和单核巨噬细胞浸润为主的细胞免疫反应,且CD4^+/CD8^+比值明显下降,但两病的局部免疫仍存在异常。 Objective: To investigate the status of innate immunity and acquired immunity in the skin lesions of condyloma acuminata and bowenoid papulosis. Methods: Immanochemistry assay and monoclonal antibodies were used to detect the expression of Toll-like receptor (TLR) 2 and TLR4, as well as the immunostaining with CD3, CD4, CD8 and CD68 in the skin lesions of condyloma acuminata and bowenoid papulosis. Results: The positive staining for TLR2 and TLR4 was seen in the specimens of both condyloma acuminata and bowenoid papulosis. Strongly positive staining for TLR2 was found in the prickle cells, basal cells of epidermis and the inflammatory cells in superficial dermis. There were marked inflammatory cell infiltration in the superficial and middle dermis, predominantly CD3^+,CD8^+ and CD68^+ cells which were much more in condyloma acuminata than those in bowenoid papulosis. Conclusions: In the prophrase, the epidermal cells and mononuclear- macrophages in both condyloma acuminata and bowenoid papulosis can recognize and process the HPV viral antigens because of the positive mark of the Toll-like receptor. We found cell-mediated immune response in the superficial and intercellular layer of derma in the local skin rashes of condyloma acuminata and bowenoid papulosis, and the mainly infiltrated cells are lymphocyte and mononuclear-macrophages, and the ratio of CD4^+/CD8^+ descended signifcantly, but the local immunosuppression between condyloma acuminata and bowenoid papulosis is different.
