经颅多普勒与闪光视觉诱发电位预测颅内压的对比 被引量:1

摘要 目的:观察经颅多普勒(TCD)血流动力学参数及闪光视觉诱发电位(FVEP)各波潜伏期与颅内压(ICP)的相关变化,对比研究2者无创预测ICP的准确性。方法:对157例颅脑病变患者腰椎穿刺测压前均行TCD与FVEP检查,依不同ICP范围分为:ICP正常组(A组,n=48)、轻度颅高压组(B组,n=52)、中度颅高压组(C组,n=57),另选30例正常人群作为对照组(D组,n=30)。记录TCD血流动力学参数[收缩期峰血流速度(vs)、舒张期末血流速度(vd)、平均血流速度(vm)、搏动指数(PI)]、FVEP各波(P2、N2、P3、N3)潜伏期及脑脊液压力。结果:与A、D组比较,B、C组vs、vm、PI、各波潜伏期差异均有统计学意义(P均<0.05)。TCD参数PI与ICP的相关系数最大(r=0.769,P<0.001),FVEP的N2波潜伏期与ICP的相关系数最大(r=0.642,P<0.001)。2种方法对ICP的预测差异无统计学意义(P>0.05)。结论:TCD及FVEP均可较准确地无创预测ICP。 Aim : To observe correlation between hemodynamic parameter of transcranial Doppler (TCD) , latent period of waves of flash visual evoked potential (FVEP) and intracranial pressure (ICP), and compare accuracy of TCD and FVEP in ICP prediction. Methods:TCD tests and FVEP examination were carried out for 157 adult suffered from encephalopathy before lumbar puncture and 30 normal subjects. According to ICP value, patients were divided into normal ICP group( A group,n = 48), mild ICH group( B group, n = 52) , moderate ICH group( C group,n = 57), and control group( D group,n =30). Hemodynamic parameter of TCD, latent period of waves of FVEP,and cerebrospinal fluid pressure were measured and analyzed. Results: Compared with A and D group,ICP increased, diastolic phase velocity (vd ) decreased, pulse index (PI) increased, and latent period of P2, N2, P3 ,and N3 prolonged in B and C group (P 〈 0.05). There was the largest correlation coefficient between PI and ICP (r = 0. 769,P 〈 0. 001 ), between latent period of N2 and ICP (r = 0. 642 ,P 〈0. 001 ). There was no significant between TCD and FVEP in ICP prediction( P 〉 0.05 ). Conclusion: Both TCD and FVEP could accuately predict ICP.
