早期应用依达拉奉对大面积脑梗死患者血清神经元特异性烯醇酶和C反应蛋白水平及病死率的影响 被引量:8

Influence of Early Using Edaravone on Serum Neuron-Specific Enolase and C-Reactive Protein Levels and Mortality in Patients with Acute Massive Hemispheric Infarction

摘要 目的观察大面积脑梗死患者早期应用依达拉奉对病死率以及血清NSE和CRP水平的影响。方法90例(发病48h内入院)急性大面积脑梗死患者,随机分为治疗组(依达拉奉+常规治疗)和对照组(常规治疗),治疗前、后进行NIHSS评分,统计病死率和检测血清NSE和CRP水平,并对两组间和治疗前、后的数据比较。结果治疗组的病死率显著低于对照组,治疗后第7、14、21d的NIHSS评分无显著差异;与对照组相比,治疗后第7、14d血清NSE水平差异显著,第21d无显著差异;第7d血清CRP水平相差显著,第14、21d则无显著差异;与治疗前相比,治疗组治疗后第7、14、21d血清NSE和CRP水平均有显著下降,对照组第14、21d有显著下降。血清NSE和CRP水平呈正相关变化。结论早期应用依达拉奉可以显著降低患者血清NSE和CRP水平,减轻脑组织损伤,并显著降低大面积脑梗死患者病死率。 Objective To observe the influence of early using edaravone on mortality and serum neuron- specific enolase and C -reactive protein levels in patients with acute massive hemispheric infarction. Methods 90 patients ( admitted within 48 hours after onset) with acute massive hemispheric infarction were divided into two groups randomly, treatment group and control group. Mortality and the scores of NIHSS were recorded, the levels of serum neuron - specific enolase and C - reactive protein were measured before and after treatment. Results Compared with the control group, mortality was decreased significantly in treatment group, the difference of NIHSS was not significant, serum NSE levels were decreased significantly at d7 and d14 but not at d21 after treatment, difference of serum CRP levels at d7 was significant, but not at dl4 and d21. Compared with stages before treatment, the serum NSE and CRP levels in treatment group were decreased significantly at d7 or d14 or d21 after treatment, in control group at d14 or d21. Conclusion Serum NSE and CRP levels and mortality in patients with acute massive hemispheric infarction can be decreased significantly by early using edaravone.
