免疫性血管炎时IL-1β、IL-6、TNF-α与血小板变化的研究 被引量:1

摘要 目的探讨炎性细胞因子IL-1β、IL-6、TNF-α在免疫性血管炎中的变化及其与血小板数和功能的关系。方法建立免疫性血管炎模型,ELISA法检测血清IL-1β、IL-6、TNF-α水平,检测外周血小板数、血小板聚集功能、血小板活化标志Annexin V,分析它们之间的相关性。HE染色、弹力纤维染色和电镜观察血管病变。结果急性期模型组炎性细胞因子IL-1β和TNF-α较正常对照组明显增高(P<0.05),细胞因子IL-6在各组之间差异无统计学意义。血小板数、血小板聚集功能和由Annexin V检测的血小板活性标志较正常对照组明显增高。相关分析显示,血小板数与IL-1β有一定程度的相关,血小板聚集功能与IL-1β、TNF-α有关,Annexin V检测的血小板活性则主要与TNF-α相关。阿司匹林治疗可明显降低上述各指标,IVIG不能降低血小板数和功能,但能降低炎性细胞因子IL-1β和TNF-α水平。两者均可明显减轻血管炎的病理损害。结论炎性细胞因子IL-1β、TNF-α参与了血管炎的发生发展,并与血小板数和功能相关。抗血小板药物可通过抑制血小板及抑制炎性细胞因子作用,从而减轻血管炎的病理损害,达到治疗目的。 Objectives To study the changes of cytokines, platelets and their relationship in immune vasculitis. Methods The rabbit models of immune vasculitis were established by twice intravenous injection of bovine serum albumin. Peripheral platelet number, platelet aggregation and Annexin V were detected. The levels of serum Interleukin-1β (IL- 1β), Interleukin-6 (IL-6), tumor necrosis factor-α(TNF-α) were detected by enzyme-linked immuuosorbent assay. The pathological changes of immune vasculitis were analyzed by hematoxylin and eosin staining, elastic fibers staining and electron microscopy. Results The levels of IL-1β, TNF-α, platelet number, platelet aggregation and Annexin V in the vasculitis group were significantly higher than those in the control group (P 〈 0.05) , while the level of IL-6 was not significantly different from that in the control group. The serum level of IL-1β was correlated with platelet number, while both serum IL-1β and TNF-α levels correlated with the platelet aggregation. TNF-α level also correlated with Annexin V. Aspirin treatment could inhibit the serum levels of IL-1β and TNF-α, platelet number and activity. IVIG could inhibit the levels of IL-1β and TNF-α but could not inhibit the number and activity of platelet. Both drugs could improve the vessel damage from immune vasculitis. Conclusions Cytokines IL-1β, TNF-α and platelet might be involved in the pathogenesis of immune vasculitis, and aspirin could decrease the inflammation damage of vessels in immune vasculitis through inhibiting cytokines and platelets.
