细胞因子、粘附分子与蛛网膜下腔出血后脑血管痉挛的关系 被引量:4

摘要 脑卒中已成为当今社会第三位的致死和致残的原因。蛛网膜下腔出血(subarachnoidhemorrhage,SAH)占所有类型脑卒中的6%~8%,并占因脑血管病死亡人数的22%~25%。尽管诊断治疗SAH有了长足进展,但其预后仍不理想,有近半数的患者死亡,大约15%留有重度残疾,只有20%~35%恢复一般或较为理想。脑血管痉挛是SAH后常见而致命的并发症,已被认为是影响预后的重要因素。SAH患者中近70%的患者在脑血管造影中发现动脉痉挛,20%.30%患者有临床表现。尽管应用最佳的治疗方案,但仍有近50%出现脑血管痉挛症状的患者最终发展为脑梗死。目前SAH后脑血管痉挛的致病机制仍不清楚,但现已进行的临床和实验室研究均表明细胞因子及粘附分子在脑血管痉挛的发生发展过程中起重要作用。
