脑肿瘤对初级运动皮层BOLD信号影响的多因素分析 被引量:3

Effect of Brain Tumors on BOLD Signal Intensity in the Primary Motor Area:a Multiple Regression Analysis

摘要 目的观察不同类型脑肿瘤对两侧初级运动区(M1)激活强度和范围的影响,分析影响运动区BOLD信号的主要因素。资料与方法选择位于幕上中央沟附近,累及或邻近M1区的脑肿瘤患者32例。按照脑肿瘤的类型、级别、部位分别进行分组比较。同时选择10名正常人作为对照组。血氧水平依赖功能磁共振成像(BOLD-fMRI)采用组块设计,同时获取脑肿瘤侧及非脑肿瘤侧M1区激活强度及范围。采用Stepwise逐步筛选法,引入脑肿瘤直径、脑肿瘤与M1区距离、M1区移位、脑肿瘤类型、分级、部位进入模型,分析影响BOLD信号的主要因素。结果按脑肿瘤类型分组的胶质瘤组、脑膜瘤组、转移瘤组、正常对照组间在激活强度上无统计学差异;在激活范围上各组间存在统计学差异。按照脑肿瘤WHO分级、部位分组,各组间在激活强度和范围上均存在统计学差异。Stepwise逐步筛选法显示,在激活范围上,有统计学意义的影响因素为脑肿瘤WHO分级和M1区移位(t=3.98,P=0.00;t=2.80,P=0.01)。在激活强度上,有统计学意义的影响因素为脑肿瘤WHO分级(t=3.00,P=0.01)。结论在脑肿瘤对M1区BOLD信号产生影响的诸多因素中,脑肿瘤的级别是影响M1区激活强度的主要因素;肿瘤级别和M1区移位程度是影响M1区激活范围的主要因素。 Objective To observe the activating differences between bilateral primary motor area ( M1 ) in patients with brain tumors using BOLD-fMRI with hand motion and to analyze the major influencing factors of the motor area BOLD signal. Materials and Methods Thirty-two patients with brain tumors located within or near the pre - or postcentral gyms were collected in the study. The lesions were divided into different groups according to the type, grade and location, of the brain tumors. 10 volunteers were collected as control group. The influencing factors, such as the type and diameter,of brain tumor, distance between the tumor and M1, displacement of the M1 on BOLD signal were analysed. The BOLD signal in M1 among different types of tumor were compared. Results There was no statisticaly significant difference on signal intensity among the different types of the tumor, while there was statisticaly significant difference on the activated extent among these groups. The significant factors on activated extent were brain tumors classification and displacement of M1, The significant factors on the activated intensity were brain tumors classification. Conclusion The major influencing factor on BOLD signal intensity was the grade of brain tumors, and the major influencing factors on BOLD signal extent were the grade of brain tumors and the displacement of the M1.
