脑心通胶囊对脑梗死患者血清肿瘤坏死因子-α及细胞间黏附分子-1水平的影响 被引量:2

The Influence of Naoxintong on Plasma Tumor Necrosis Factor-alpha and Soluble Intercellular Adhesion Molecule-1 in Acute Cerebral Infarction Patients

摘要 目的观察脑心通对脑梗死患者血清肿瘤坏死因子-α(TNF-α)及可溶性细胞间黏附分子-1(sICAM-1)水平的影响,并探讨其作用机制。方法150例急性脑梗死患者随机分为脑心通治疗组75例和对照组75例,采用双抗体夹心酶联免疫吸附法(ELISA)测定发病后第1天、第3天、第7天、第14天血清TNF-α和sICAM-1水平的变化。在治疗前和治疗后第14天进行欧洲卒中评分(ESS)及日常生活能力量表(ADL)评分,治疗第14天的ESS和ADL评分增分率判断疗效。结果治疗后7d和14d,治疗组血清TNF-α和sICAM-1水平明显低于对照组,差异有统计学意义(P<0.01)。治疗组ADL增分率高于对照组,差异有统计学意义(P<0.05);两组ESS增分率差异无统计学意义(P>0.05)。治疗前两组患者血清TNF-α水平均与ESS和ADL评分呈负相关(r值分别为-0.436、-0.438,P<0.001);血清sICAM-1水平均与ESS和ADL评分呈负相关(r值分别为-0.453、-0.461,P<0.001)。结论脑心通可降低急性脑梗死患者血清TNF-α和sICAM-1水平,改善患者的生存质量。 Objective To observe the influence of Naoxintong on plasma tumor necrosis factor - alpha ( TNF - α) and soluble intercellular adhesion molecule - 1 ( sICAM - 1 ), expore its mechanism. Methods One hundred and fifty patients suffering from acute cerebral infarction were divided randomly into Naoxintong trial group ( n = 75 ) and control group ( n = 75 ). Before and at 3rd, 7th, 14th day after treatment, serum TNF - α and sICAM - 1 levels were detected by double - antibody - ELISA. Before and at 14th day after treatment, the patients were evaluated by European stroke scale (ESS) and activities of daily living ( ADL), and the changes of ESS and ADL were served as the index of efficacy. Results Compared with those of control group, serum TNF - α and sICAM - 1 levels were a significant decrease at 7th and 14th day after treatment ( P 〈0.01 ). The increasing ratio of ADL in naoxintong group was higher than those control group, the difference is significant ( P 〈 0.05 ). but there was no obvious difference in the increasing ratio of ESS ( P 〉 0. 05 ). Linear correlation analysis showed a predominant negative correlation between TNF - cdevel and the score of ESS and ADL in all ACI patients before treatment ( r = - 0.436, r = - 0. 438, respectively, P 〈 0. 001 ). It is similar between sICAM - 1 level and the score of ESS and ADL ( r = - 0. 453, r = - 0. 461, respectively, P 〈 0. 001 ). Conclusion Naoxintong capsule may decrease the serum level of TNF - α and sICAM - 1, improve the prognosis in the acute cerebral infarction patients.
