重型肝炎患者血浆置换治疗对血清细胞因子的变化

Serum cytokine changes in patients with severe hepatitis after plasma exchange treatment

摘要 目的:观察重型肝炎患者在血浆置换治疗前后血清IL-2,IL-6,TGFβ1,TNF-α,sFas及IFN-α水平的变化.方法:采用血浆置换技术治疗重型肝炎58例,用ELISA法测定治疗前后血清中IL-2,IL-6,TGFβ1,TNF-α,sFas及IFN-α的水平,并与20例正常献血员进行对照.结果:治疗组患者治疗前血清IL-2,IFN-α水平明显低于对照组(P<0.01),IL-6,TGFβ1,TNF-α及sFas水平明显高于对照组(P<0.01);治疗后血清IL-2,IFN-α水平较治疗前明显升高(P<0.01),IL-6,TGFβ1,TNF-α及sFas水平较治疗前明显下降(P<0.01).结论:血浆置换对IL-6,TGFβ1,TNF-α及sFas有较好的清除作用,可使IL-2和IFN-α水平升高,能减轻重型肝炎时肝细胞的损伤,提高重型肝炎的存活率. AIM: To investigate serum IL-2, IL-6, TGF β1, TNF-α, sFas, IFN-α levels in patients with severe hepatitis before and after plasma exchange treatment. METHODS: Plasma exchange technology was ased to treat 58 cases with severe hepatitis. Serum IL-2, IL-6, TGF β1, TNF-α, sFas, IFN-α levels were deter mined by ELISA before and after treatment, which were compared with 20 normal blood donors. RESULTS: Before treatment, serum IL-2, IFN-α levels were significantly lower ( P 〈 0. 01 ), and IL-6, TGF β1, TNF-α, sFas levels were significantly higher in treatment group than in control group (P 〈 0.001 ). After treatment, serum IL-2, IFN-α levels increased significantly (P〈0.01), and IL-6, TGF β1, TNF-ct, sFas levels decreased significantly (P 〈 0. 01 ). CONCLUSION: Plasma exchange play an important role in removing IL-6, TGF 131, TNF-α, sFas, which can increase IL-2, IFN-α level, reduce liver cell damage in severe hepatitis, and improve the survival rate of severe hepatitis.
