摘要
目的:了解我院门诊不合理用药情况,分析原因,阐述机理,促进合理用药。方法:根据药品说明书、临床药理学知识及文献资料,对我院2007年1-12月门诊不合理用药处方进行分析。结果:不合理用药处方占所审核处方的3.42%,不合理用药主要反映在药物配伍不合理、重复用药、给药方法不合理、选用溶媒不合理、选药不合理方面。结论:这些不合理用药现象主要是临床医生对药物知识的缺乏所致,药学人员应向临床药师职能转化,向医生提供更多的药学知识,指导医生和病人合理用药,以保障患者的用药安全。
Objective：To explain the condition of irrational prescriptions,analyze its reasons and comprehend its mechanisms,promote the rational drug use.Methods：According to medicine instructions,clinical knowledge and documents,we analyzed the incorrect prescriptions from January to December of 2007.Results：The ratio of unreasonable prescriptions among the total was 3.42% and the main problems lay in the unreasonable compatibility,repeat medication,incorrect medication,unreasonable dissolvent and incorrect medicine.Conclusion：Lack of clinical knowledge is the main reason,so the pharmacist should develop the function of clinical pharmacist,then provide more pharmaceutical knowledge for clinicians in order to guide the rational drug use and make sure the safety of the drug use.
