摘要
目的研究阿霉素纳米微粒经舌动脉超选择性插管给药后,药物在靶器官及易损器官内的分布特点,探讨阿霉素纳米微粒治疗舌癌的靶向性。方法将40只荷舌癌的纯种新西兰白兔随机分成阿霉素纳米微粒组和阿霉素组,每组20只,以超选择性舌动脉插管分别注射阿霉素和阿霉素纳米微粒。每组给药后1h、5h、15h各处死5只兔,分别取舌、舌癌、心、肝、脾、肺、肾组织各1.0g,血浆1mL,用高效液相色谱法检测组织和血浆内的药物浓度。给药45d后切取兔舌癌组织,制作连续病理切片,检测治疗效果。结果给药后1h、5h、15h,阿霉素纳米微粒组舌癌内的药物浓度显著高于阿霉素组(P<0.01),而血浆、心、肺中阿霉素浓度则显著降低(P<0.01);阿霉素纳米微粒组中,给药后1h、5h、15h以舌癌和舌组织内药物浓度为最高,各时间点均以心脏药物浓度为最低。45d后阿霉素纳米微粒组均达到完全缓解,病理切片仅见少量癌细胞。结论阿霉素纳米微粒经舌动脉超选择性插管给药后,可以显著提高阿霉素在舌癌病灶内的浓度,具有显著的靶向治疗效果。
Objective To examine the distribution of adriamycin nanometer particles in target organ (tongue cancer) and easy-damaged organs by super-selective lingual arterial intubation. Methods Forty pure-bred New Zealand rabbits inoculated with tongue squamous cell carcinoma were randomized into adriamycin group and adriamycin nanometer particles group, 20 animals each. Adriamycin and adriamycin nanometer particles were administered to these two groups respectively through super-selective lingual arterial intubation. 1 h, 5 h and 15 h after administration, 5 rabbits in each group were sacrificed and 1.0 g tissue of tongue, carcinoma, heart, liver, spleen, kidney and lml of plasma were collected, and the drug concentrations were examined by high performance liquid chromatography ( HPLC ). 45 days after drug administration, pathological examination of the samples was performed to evaluate the therapeutic effects. Results The drug concentration in tongue carcinoma of adriamycin nanometer particles group was siguificantly higher than that of free adriamycin group (P 〈0. 01 ), whereas much lower in non-target organs( heart, liver, spleen, kidney and plasma) (P 〈0. 01 ). 1 h, 5 h and 15 h after administration, the drug concentration was highest in tongue and carcinoma tissue and lowest in heart in nanometer particles group. Pathology showed that only little cancer cells were found. The therapeutic effect of nanometer group attained CR. Condusion The super-selective lingual arterial administration of adriamycin nanometer particles can markedly enhance drug concentration in the cancer lesion and has better treatment effect.
