摘要
目的通过观察2型糖尿病(Type 2 diabetes mellitus,T2DM)合并血脂异常患者尿微量白蛋白的患病率,探讨脂代谢异常与T2DM患者尿中微量白蛋白(Microalbunm inuria,MAU)之间的关系。方法选择T2DM合并血脂异常患者133例,尿微量白蛋白测试条半定量测定MAU,根据检测结果分为阴性、微量及大量白蛋白尿组,测定各组患者体重指数、腰围、血压、空腹血糖、血脂。结果在133例T2DM合并血脂异常患者中MAU及大量白蛋白尿的发生率分别为27%和22.6%,微量及大量组较阴性组TG显著增加(P<0.01)。结论T2DM合并血脂异常患者中有约50%的患者存在肾损害,推测从尿白蛋白阴性发展为MAU的过程中,高TG加强了高血糖引起的肾损害。
Objective To search for the relationship between the lipid metabolism imbalance and microalbunminuria （MAU） in urine in type 2 diabetes mellitus （T2DM） through the investigated the prevalence rate of MAU in T2DM patients who complicated the lipid metabolism imbalance. Methods 133 cases who were T2DM with lipid metabolism imbalance were examined for MAU by using commercially available test strips. The patients were divided into three groups which were the normal group, Microalbuminuria group, and macroalbuminuria group according to MAU test result. The fol- lowing targets which were the body mass index, waist circumference, blood pressure, and blood fat were measured for all patients. Serum fasting blood glucose and total cholesterol （TC）, triglyceride （TG）, high density lipoprotein-cholesterol （HDL-C）, low density lipoprotein-cholesterol （LDL-C） were detected by TOSJIBA TBA-40FR automatic biochemical analyzer. Results The rate of microalbuminutia anti macroalbuminuria were 27% and 22. 6% respectively. TG in microalbuminuria group and macroalbuminuria group were significantly increased compared with that in normal group （P〈0. 01）. Condusion Nearly 50 percent of type 2 diabetic patients with lipid metabolism imbalance have renal damage, maybe exorbitant TG increase renal damage.
