摘要
选择2005-08/2007-04收治的根据Geoffroy和Harding等制定的标准及DNA检测确诊的Friedreich共济失调患者6例,男1例,女5例,年龄14~28岁,病程5-14年。患者及家属对治疗知情同意,并经医院伦理委员会批准。4例于全麻下手术,选取脊髓变细处上、中下3点或2点为细胞注射部位,每例注射嗅鞘细胞2×10^6。2例于局麻下手术,选取双顶放射冠为注射细胞靶点,每侧注射嗅鞘2×10^6。术前和术后2~4周采用世界神经病联合会国际合作共济失调量表比较其治疗结果,分数越高表示神经功能损害越严重。采用SPSS13.0统计软件进行数据处理及分析,2~4周后患者均有不同程度的神经功能改善,6例患者世界神经病联合会国际合作共济失调量表评分较术前下降(P=0.009),改善主要表现在行走能力、步态、站立平衡方面。术后无发热、头痛、头晕、恶心及呕吐等神经系统并发症发生。对2例患者进行半年以上的远期随访,平衡及行走功能继续改善,例1共济失调量表评定总分术前38分降至35分,术后随访7个月,共济失调量表评分降至22分。
Between August 2005 and April 2007, six patients (one male and five females) with Friedreich's ataxia diagnosed by Geoffroy and arding standards and DNA were enrolled. Their ages ranged from 14 to 28 years. The duration of course was 5 to 14 years. All the patients signed the informed content and the clinical study was approved by Medical Ethics Committee. Olfactory ensheathing cells (2×10^6) were transplanted into the diseased region of the spinal cord in 4 patients after general anesthesia and the corona radiata of the parietal lobe in both sides in 2 patients. The neurological function was assessed before and 2 to 4 weeks after the surgery using International Cooperative Ataxia Rating Scale (ICARS) by World Federation of Neurology. The higher score presents severe neurological function. All data were analyzed with SPSS 13.0 software. 2 to 4 weeks later, all patients obtained some neurological amelioration to different degrees. ICARS score decreased than before the surgery (P=0.009). Locomotor activity, walking and standing balance were improved. No complications were detected after the surgery, such as fever, headache, dizziness, nausea and vomiting. 2 patients were followed up for more than six months. Balance and locomotor activity had the trend of improvement. Patient 1, the ICARS score decreased from pre-operative 38 points to post-operative 35 points. During the period of 7-months follow-up, the ICARS score decreased to 22 points.
