摘要
目的:探讨5-FU门静脉化疗对结直肠癌术后癌细胞血液微转移的影响。方法:研究组27例,结直肠癌根治术后接受门静脉化疗,采用巢式RT-PCR方法检测血液中CK20mRNA;同时取23例单纯接受结直肠癌根治术患者作为对照组。结果:研究组和对照组术后外周静脉血中CK20mRNA的阳性率均较术前显著升高,差异有统计学意义(P<0.05);两组术后即刻抽取的门静脉血和外周静脉血之间CK20mRNA阳性率的差异均无统计学意义(P>0.05);经门静脉化疗后,术后第8、30天外周静脉血CK20mRNA阳性率较术后(即刻)下降,差异有统计学意义(P<0.05);而单纯接受结直肠癌根治术的患者,术后第8、30天外周静脉血CK20mRNA阳性率与术后(即刻)相比,差异无统计学意义(P>0.05)。结论:手术操作可能会促进结直肠癌细胞的血液微转移,而术后早期5-FU门静脉连续化疗可以降低血液微转移的风险。
Objective：To investigate the influence of portal vein chemotherapy with 5 - FU for colorectal cancer on postoperative hematogenous micrometastasis of tumor cells. Methods： Peripheral blood samples were collected from 27 patients with portal vein chemotherapy after curative resection of colorectal cancer, and other 23 patients without portal vein chemotherapy as control. CK20mRNA was detected by nested reverse transcriptase -polymerase chain reaction（ RT- PCR）. Results： The positive expression rate of CK20mRNA in postoperative peripheral blood samples was significantly higher than that in preoperative blood samples for both test group and control group （ P 〈 0. 05 ）. The positive expression rate of CK20mRNA between postoperative blood samples of portal vein and peripheral vein showed no significant difference （ P 〉 0. 05 ）. With portal vein chemotherapy, the positive expression rate of CK20mRNA on the 8th, 30th day after operation were lower than that of instant after operation （P 〈 0.05 ）. There was no significant difference on the 8th, 30th day after operation and instant after operation for control group （P 〉 0.05 ）. Conclusion： Operation of colorectal cancer may increase the risk of hematogenous tumor micrometastasis.Portal vein chemotherapy with 5 - FU for colorectal cancer after resection may decrease the risk of hematogenous tumor metastasis.
