摘要
目的:探讨食管癌根治术的主要并发症及其预防措施。方法:分析689例食管癌根治术后主要并发症发生情况,总结经验教训,探讨预防措施。结果:食管癌根治术后并发症总发生率为22.4%(155/691),其中肺部并发症16.8%,吻合口瘘5.1%,乳糜胸1.0%,脓胸0.7%。并发症死亡率为14.8%(23/155),因肺部并发症死亡者占43.4%(10/23),因吻合口瘘死亡者占21.7%(5/23)。结论:食管癌根治术后并发症仍以肺部并发症、吻合口瘘多见,且为主要死亡原因。做好术前准备,严格规范手术操作,是预防并发症的主要措施。
Objective:To discuss the main complications in radical surgery for esophageal carcinoma and the prevention. Methods: The clinical data of 689 patients with esophageal carcinoma admitted to our department from 1996 to 2007 treated with radical esophagectomy were analyzed. Results : The incidence of the complications was 22.4%. The main complications were as follows: pulmonary complications ( 16.8% ), anastomotic leak (5.1% ), chylothorax ( 1.0% )and empyema(0.7% ). The hospital mortality rate was 3.3%. The main death causes were pulmonary complications(43.4% ) and anastomotic leak(21.7% ). Conclusion: The major complications following radical surgery for esophageal carcinoma are pulmonary complications and anastomotic leak. They are also the main death causes. Making a good preoperative preparation, standardizing the operative procedure are the critical measures of preventing the complications.
