摘要
目的:探讨血清中可溶性白细胞介素-6受体(soluble interleukin 6 receptor,sIL-6R)水平在胃癌患者手术前后的变化及临床意义。方法:采用ELISA方法检测30例正常人、30例胃癌患者手术前后可溶性sIL-6R,比较手术前后sIL-6R水平。结果:胃癌患者血清sIL-6R水平为(62.32±7.32)μg/L,明显高于对照组(41.83±5.70)μg/L(P<0.01);Ⅳ和Ⅲ期胃癌为(69.94±9.33)μg/L,明显高于Ⅱ和Ⅰ期的sIL-6R(52.34±7.83)μg/L(P<0.01);高、中、低分化胃癌sIL-6R水平分别为(52.99±6.37)μg/L(、60.33±7.12)μg/L、(69.06±5.83)μg/L,不同分化程度间均有显著性差异(P<0.01);胃癌患者术后为(39.60±7.60)μg/L,显著低于术前(P<0.01);sIL-6R水平与临床分期、细胞分化程度有关,与胃癌患者性别、年龄无关。结论:胃癌患者体内存在着sIL-6R的异常表达,手术后血清sIL-6R水平低于手术前,血清sIL-6R水平可能与胃癌的发生和发展有关,手术可抑制肿瘤细胞的增殖和生长,动态检测sIL-6R水平对判断胃癌病情和评价疗效有一定的参考意义。
Objective：To investigate the changes of serum sIL - 6R in patients with gastric cancer before and after operation and its clinical significance. Methods：The level of serum sIL - 6R was measured by ELISA in 30 normal subjects and 60 cases of gastric cancer before and after operation, and the changes of sIL - 6R were compared before and after the operation in the patients with the gastric cancer. Results：The level of serum sIL - 6R increased in all patients （62. 32±7. 32）μg/L and was higher than that in the normal group （41.83±5. 70）μg/L（P〈0. 01）. The level of serum sIL - 6R in the patients with gastric cancer in stage Ⅳ and Ⅲ （69. 94±9.33）μg/L was higher than that in stage Ⅱ and Ⅰ （52. 34±7. 83）μg/L （P〈0. 01）. The level of serum sIL - 6R in well differentiation, moderate differentiation and poor differentiation was （52. 99±6. 37） μg/L, （60. 33± 7. 12） μg/L and （69. 06± 5. 83） μg/L, respectively. After the operation treatment, the level of serum sIL - 6R in patients with gastric cancer declined significantly. The level of serum sIL - 6R was correlated with the clinical stage and histological grade of the cancer, not correlated with sex and age. Conclusion： Patients with gastric cancer have an abnormal expression of sIL - 6R. The level of serum sIL - 6R decreases after the operation, which may be involved in the occurrence and development of gastric cancer.
