摘要
目的探讨代币疗法在慢性精神病人康复中的作用。方法将89名慢性精神病人分为2组,即代币组(n=47)和对照组(n=42),对代币组实施代币疗法,并在实施前后采用PANSS量表、NOSIE及康复量表与对照组患者进行评定比较。结果实施代币疗法后,代币组PANSS量表评定除一般精神病理、反应缺乏外,阳性、阴性量表分及各因子分均明显低于实施前(p<0.05或0.01);NOSIE评定社会能力、社会兴趣、个人整洁、总积极因子及总分得分明显高于实施前(p<0.01);激惹、精神病表现、抑郁、迟缓、总消极因子得分明显低于实施前(p<0.01)。代币组社会兴趣、个人整洁、总积极因子得分高于对照组,精神病表现低于对照组;康复量表评分明显高于对照组(p<0.05)。结论代币法能在整体上改善精神病人的精神状况,改变病人的一些行为习惯,在激发病人主观能动性效果方面优于单纯药物治疗。
Objective： To explore the effects of token therapy on chronic psychotic during recovery training. Methods： 47 chronic psychotric patients（experiment goup） given token therapy were assessed using PANSS scale, NOSIE scale and Restoration Assessment scale before and after one month. 42 cases（control group） given drug treatment was compared with experiment group using NOSIE scale before and one month later. Results： Except the facors of general psychopathology and lack of reaction, the scores of other facors of PANSS and positive and negtive scale were all lower after one month（p〈0.05 or p〈0.01）; the scores of factors in NOSIE, such as social capacity, social interest, individual clean and tidy, and the total score of positive factors were all higher one month later（p〈0.01）, but the scores of irritation, symptoms manifestitation, depression, retardness, and total score of negtive factors were all lower than the beginning（p〈0.01）.Compared to control group, the scores of social intetrest, individual clean and tidy and positive factors were higher and psychotic manifestitaion was lower in experiment goup. The effects of recovery in the experiment group was better than that in the control group.Conclusion： Token economy can generally improve the patients＇ psychology status and some behavior, and can also stimulate themselves＇ motivation.
