摘要
目的:探讨支撑吻合管环扎吻合在腹腔镜直肠癌切除术中的应用。方法:回顾性分析了2005年12月～2006年7月9例施行腹腔镜环扎吻合术的直肠癌患者的临床资料。结果:全组手术平均耗时170min;术后胃肠功能恢复时间1～3d;所有标本远近切缘无癌残留。术后肛诊吻合口愈合良好,肛门括约功能存在,患者早期即有排便感觉,但控便能力较差,经肛门功能锻炼后,控便能力恢复,排便(3～5)次/d,且对气体或粪便能够明确鉴别。无术后出血、吻合口狭窄和吻合口漏等并发症。结论:腹腔镜直肠癌切除支撑吻合管环扎吻合术简便易行,且能够取得其他吻合方法难以达到的支撑和引流效果。
OBJECTIVE： To introduce the application and performance of anus-preserving operation by using supporting tube in laparoscopic resection of rectal cancer. METHODS：The clinical data of 9 cases of anuspreserving operation by using supporting tube in laparoscopic resection of rectal cancer from December 2005 to July 2006 were reviewed retrospectively. RESULTS： Total of the operations were performed successfully. The average duration of operation was 170 min, and the gastrointestinal function recovered within 1-3 days. There was no tumor cell residual at the top and the bottom line of the resected specimens upon pathology. The postoperative examination showed excellent heal of stoma and the existence of sphincter function. The patients could get a feeling of excreting early, but they didn＇t control it well before practicing to excrete 3-5 times a days. They could also differentiate excrement from gas. None in this series developed complications such as bleeding, narrowness and anastomosis leakage. CONCLUSION： The anus-preserving operation by using supporting tube in laparoscopic resection of rectal cancer is easy to carry out, and this surgical procedure can acquire incomparable effect of draining and supporting.
