沙门菌检测在临床粪便样品的优化方法应用 被引量:12

摘要 目的:建立有效增菌的优化检测方法强化腹泻病人的沙门菌型监测。方法:评估CAS、XLD直接平板分离和与SBG增菌液的组合应用检测腹泻病人并同时与常规SF增菌液-SS平板分离的效果比较。结果:2996份肛拭经SBG增菌后用CAS、XLD分离的菌株数和敏感性分别为115株、92.74%和90株、72.58%,均高于CAS、XLD直接分离的21株、16.94%和19株、15.32%,与SF-SS的常规分离方法的31株,25.00%有显著性差异(P<0.005);总阳性分离率4.14%,最高峰值在9月份是5.95%;肠炎、山夫登堡、鼠伤寒沙门菌是优势的致泻血清型。结论:SBG增菌液和双平板联用的优化方法大大提高常规平板分离的敏感性,筛选特异性好,技术简单易操作,在以临床实验室为基础的症状监测中应用。 Objective:To establish an optimal method of effective enrichment for enhanced systemic surveillance Salmonella serotype from fecal specimens of diarrhea patients. Methods: To evaluate the sensitivity by using CAS and XLD direct -plating method and after enrichment with SBG broth recovery of Salmonella from fecal speciments of diarrhea patients, meanwhile the classical enrichment methods for the isolation of Salmonella have been compared. Resultes: Examination of 2996 clinical fecal swab, final 115 and 90 Salmonella spp. were isolated that sensitivities were 92. 74% and 72. 58% , respectively using SBG broth inrichemnt subclture onto CAS and XLD agar, all of them were higher than the results of direct - plating onto CAS and XLD which were 21 and 19 isolates, 16. 94% and 15.32% and SF - SS method which were 31 isolates ,25.00% ( P 〈 0. 005 ). Total isolation rates of Salmonella was 4. 14% in 2996 clinical fecal swab,the highest of isolation rate in September(5.95% ). Salmonella serotype Enteritidis, Senftenberg and Typhimurium were dominant serotypes causing diarrhea. Conclusion :The optimal method which SBG broth and the combination of XLD and CAS agar can increase sensitivity of conventional plating isolation methods, enhanced specificity of plating- creening, technique and operation simply, especially suggests that Salmonellosis symptom surveillance is appropriate to be used in clinical laboratories.
