摘要
目的探讨横裁包皮带蒂岛状皮瓣尿道成形术(Duckett术)一期修复先天性尿道下裂的优点及提高疗效的措施。方法采用Duckett术式一期修复尿道下裂50例,其中6例联合应用以尿道外口为基底的阴囊矩形皮瓣尿道成形术(Duplay术式)。结果40例一期治愈;8例出现尿瘘,其中3例自愈,5例再经尿瘘修补术后痊愈。2例尿道狭窄,经扩张后痊愈。结论Duckett术一期修复伴阴茎下弯尿道下裂,取材方便,皮瓣成活率高,并发症少,外形美观,适用于大部分类型的尿道下裂;娴熟的手术技巧是提高疗效的关键。
Objective To investigate the merit of one - stage repair of hypospadias using transverse preputial pedicled flap （ Duckett＇s operation）. Methods 50 children with hypospadias were treated with one - stage urethroplasty using Duckett＇s operation; Duplay urethroplasty by using scrotum rectangular skin flap with urethra orifice as the base was also performed in 6 patients. Results Primary healing achieved in 40 patients. Urinmy fistula developed in 8 patients, 3 of them spontaneously healed and the other 5 cured with a second operation. Urethral stricture developed in 2 patients and was cured with urethral dilation. Conclusion Duckettg operation is an effective operation for repair of hypospadias.
