摘要
目的:探讨早期卵裂、胚胎形态学和生长速率联合评分预测胚胎发育潜能的价值。方法:96个IVF-ET周期分为两组:第1组包括35个IVF-ET周期,该组第3日(移植日)胚胎形态学和生长速率作为选择胚胎移植的标准(下称简单评价法);第2组包括61个IVF-ET周期,该组于受精后25~26 h对胚胎早期卵裂情况进行第1次评价,第3日对胚胎形态学和生长速率再次进行评价,两次评价的综合结果作为选择胚胎移植的标准(下称联合评价法),比较两种方法的胚胎种植率和妊娠率。结果:发生早期卵裂的胚胎在移植日的优质胚胎率显著高于未发生早期卵裂的胚胎(P〈0.001);联合评价法所选择胚胎的临床妊娠率和胚胎种植率均明显高于简单评价法(P〈0.05)。结论:早期卵裂是预测胚胎发育潜能的一个有价值的指标,胚胎发育的联合评价有助于提高助孕成功率。
Objective: Exploring predictive value on embryo development potential using a combined score for early cleavage, embryo morphology and growth rate. Methods.. A total of 96 IVF - ET cycles were divided into two groups : The first one included 35 IVF - ET cycles, the selection of the embryos was based on only their morphology and growth rate on day 3 ( the transfer day) in this group. The second group included 61 IVF - ET cycles, in which the combined score system was applied. Early cleavage was evaluated first at 25 - 26h post - insemination, embryo morphology and growth rate were re - evaluated at the third day, the selection of the embryos was based on early cleavage at 25 - 26h post - insemination and embryo morphology and growth rate on day 3. Pregnancy rates and implantation rates were compared between the two methods. Results: Top quality embryo formation rate was higher in the early cleavage group than in the no early cleavage group (43. 52% versus 21. 35%, P 〈 0. 001 ). Pregnancy rates and implantation rates of embryos selected by combined score system were higher than that by evaluation of embryo on day 3 (P 〈 0. 05 ) . Conclusion: Early cleavage is an valuable parameter predicting embryo development potential, combined evaluation of embryo development may be helpful to improve pregnancy rates and implantation rates.
