摘要
目的探讨肌动蛋白和PTEN蛋白在乳腺癌腋窝淋巴结中从单个肿瘤细胞或者肿瘤细胞团到明显肿瘤细胞转移发展过程中的意义。方法收集92例女性乳腺癌术后淋巴结共293枚,采用免疫组化的方法对常规HE染色未发现转移的淋巴结行细胞角蛋白19(CK19)以及肌动蛋白染色,筛选出含有微转移、孤立肿瘤细胞的淋巴结。对筛选出的淋巴结行PTEN检测,观察不同转移情况下PTEN的表达情况。结果所有293枚淋巴结中,有明显转移的84枚(28·67%),有微转移的42枚(14.33%),含孤立肿瘤细胞者22枚(7.51%);所有明显转移灶周围均有肌动蛋白表达,34枚(80·95%)含有微转移的淋巴结中微转移灶周围有肌动蛋白表达。PTEN在84枚含有明显转移的淋巴结中均呈低表达,在38枚含有微转移的淋巴结中呈低表达,其余4枚未见PTEN表达。结论淋巴结内间质反应中肌动蛋白表达变化在乳腺癌细胞进入淋巴结发展为微转移灶及进一步发展为明显转移形成的过程中可能发挥重要的作用。PTEN蛋白的低表达可能是促进淋巴结内肿瘤细胞生长和侵袭的重要原因之一。
Objective To explore the expression and significance of smooth muscle actin (SMA) and PTEN in lymph node metastasis, micrometastasis and isolated tumor cells (ITCs) of breast cancer. Methods Two hundred and ninety-three lymph nodes were collected from the axillary samples of 92 cases of breast cancer. Immuohistochemical method was used to detect the expressions of CK19, SMA and of PTEN. Results Of 293 lymph nodes, metastasis was found in 84 nodes (28.67%), micrometastasis in 42 nodes ( 14.33% ) and ITCs in 22 nodes (7.51%). The SMA expression was found in all the tissues around metastases and 34 nodes (80.95%) of micrometastases. Low expression of PTEN was found in all metastatic nodes. Of 42 nodes of micrometastases, low expression of PTEN were found in 38 nodes. Conclusion The variation of actin in stromal reaction of axillary lymph nodes may play a key role in the development from the isolated tumor cells in lymph nodes into micrometastasis or metastasis. The low expression of PTEN may enhance the invasion and metastasis of tumor cells.
