输尿管镜下钬激光碎石术后全身炎性反应综合征相关因素分析 被引量:11

摘要 目的:分析输尿管镜钬激光碎石术后全身炎性反应综合征(SIRS)发生的危险因素,寻找更加有效和安全的输尿管镜手术途径。方法:回顾性分析输尿管镜下钬激光碎石术294例,分别对输尿管镜手术时间、术前有无尿路感染、结石负荷、双侧结石或单侧结石、性别和年龄等因素与SIRS发生的关系进行比较分析。结果:术前有尿路感染及结石负荷较大的患者术后SIRS发生率明显增高,而手术时间、性别、年龄等因素对SIRS发生率无显著影响。结论:由于手术途径及结石的客观因素,输尿管钬激光碎石术有发生SIRS的可能,术前充分准备,术中规范操作及低压灌注,术后早期诊断治疗是降低手术SIRS的有效措施和途径。 Objective:To explore a more effective and safer therapeutic strategy, we analyzed the risk factors causing postoperative systemic inflammatory response syndrome (SIRS) in ureteroscopic holmium laser lithotripsy. Methods:A retrospective clinical study was performed on 294 patients underwent ureteroscopy and holmium laser lithotripsy. Risk factors such as operation time, urinary tract infection history, stone burden, unilateral or bilateral stone, sex and age, were analyzed to assess statistical association with the occurrence of postoperative SIRS. Results : Patients with urinary tract infection history or heavier stone burden were found to have elevated occurrence of SIRS. Operation time, unilateral or bilateral stone, sex and age correlated poorly with the occurrence of SIRS. themselves are potential factors to cause SIRS.
