摘要
目的探讨输尿管镜下钬激光治疗输尿结石并发症的原因及预防措施。方法回顾性分析对2005年8月至2006年3月136例输尿管镜下钬激光治疗输尿管结石出现11例并发症发生的原因及治疗措施。结果11例并发症中输尿管黏膜撕脱2例,输尿管穿孔4例,输尿管下隧道1例,发热4例；发生率8．1%。并发症的发生与输尿管狭窄,激光击穿输尿管管壁、导管刺穿输尿管黏膜等有关。均经相应治疗获愈。结论输尿管镜下的钬激光治疗输尿道结石、安全、有效、微创。但如果操作不当则会引起严重后果。提高操作技能及掌握相应的处理措施是降低输尿管镜下钬激光治疗泌尿系结石并发症的关键。
Objective To discuss the causes of and countermeasures against complication of uretero]ithiasis treated with holmium:YAG laser under uroteroscope. Methods The 11 cases with ureterolithiasis treated with laser under uroteroscope were analyzed between Aug 2005 to Mar 2006. Results Of all the cases,two cases with ureteral mucosa evulsion,4 cases with ureteral perforation,1 case with subureteral tunnel,and 4 cases with post-operation infection all healed after treatment. Conclusions Treatment of ureterolithiasis with holmium:YAG laser under ureteroscope is safe,effective and microinvasive.But if ma- nipulation is uncorrect would be the heavy sequelae happened.To height the skill in operation and to know the correct manipulations are the keys to reduce complication of ureterolithiasis treated with ureteroscope.
