摘要 目的:研究吸入0.008%一氧化氮(NO)对肺挫伤的治疗作用.方法:通过特制多功能撞击台撞击兔右侧胸壁,建立兔肺挫伤模型,并观察吸入NO对肺挫伤病理生理变化的影响.32只新西兰纯种兔分为4组,每组8只:A组为肺挫伤组;B组为肺挫伤加NO吸入组;C组为正常对照组;D组为单纯NO吸入组.结果:①A组和B组动物与C组相比,伤后肺动脉压(mPAP),肺血管阻力(PVR)及肺内分流(Qs/Qt)明显增加,而动脉血氧分压(PaO2)下降(P<0.01).②B组吸入0.008%NO气30min后,与吸入前及A组相比能降低mPAP,PVR和Qs/Qt(P<0.01),提高PaO2(P<0.05),而静脉血高铁血红蛋白升高不显著.③单纯吸入0.008%NO气体对正常动物无明显影响.结论:吸入0.008%NO对肺挫伤有一定治疗作用. Objective: To observe the effect of inhaled nitric oxide (0.008%) on pulmonary contusion. Methods: A rabbit model of pulmonary contusion was established with the impactor and the pathophysiologic changes were observed before and after inhaling NO. Thirty two New Zealand white rabbits were divided into 4 groups (group A, B, C and D): pulmonary contusion group ( n =8); pulmonary contusion with NO inhalation group ( n =8); normal control group ( n =8) and simple NO inhalation group ( n =8). Results: Group A and B show increased pulmonary vasoconstricition, pulmonary arterial pressure, physiologic shunt and decreased partial pressure of oxygen in artery (Pa O 2 ) compared with group C ( P <0.01). Group B is characterized with decreased pulmonary artery pressure, pulmonary vascular resistant index, pulmonary physiologic shunt ( P <0.01) and increased Pa O 2 ( P <0.05) compared with NO preinhalation and group A. Conclusion: Inhaled NO exerted beneficial effects on pulmonary arterial hypertension and oxygenation induced by pulmonary contusion.
