摘要 目的:探讨P物质(SP),神经肽Y(NPY)在分泌性中耳炎发病过程中的作用.方法:阻塞咽鼓管建立分泌性中耳炎模型.用免疫组化ABC-GDN及图像分析技术研究中耳粘膜SP,NPY的分布及含量的变化.结果:中耳粘膜SP免疫反应(SP-IR)阳性纤维定位分布多与血管无关,NPY免疫反应(NPY-IR)阳性纤维多伴中小动脉走行.咽鼓管阻塞8h后,中耳即有明确渗出,中耳粘膜SP-IR阳性纤维24h开始增粗、数量增多,至72h增多更明显,染色强度加深.7d组中耳粘膜SP阳性纤维仍明显增粗,数量与正常无显著差别.中耳粘膜SP-IR阳性产物吸光度变化与渗液量呈正相关(r=0.9754,P<0.05),分泌性中耳炎对中耳粘膜NPY-IR似无影响.结论:SP可能是分泌性中耳炎的重要炎性介质,中耳积液可能与NPY无关. Objective: This experiment aimed to investigate the effects of substance P (SP) and neuropeptide Y (NPY) on the invasion of secretory otitis media. Methods: Obstructing eustachian tube was used to establish the model of secretory otitis media. Immunohistochemistry ABC GDN and image analysing technology were adopted to study the distribution and content changes of SP and NPY in middle ear mucosa. Results: The distribution of positive SP IR fibrae of SP immunoreaction in middle ear mucosa seemed not to be related to blood vessels. Most positive NPY IR fibrae were accompanied by medium or small arteries. Middle ear exudation was observed 8 h after obstruction. Positive SP IR fibrae appeared to increase 24 h after obstruction. To 72 h, positive SP IR fibrae increased obviously in quantity and in staining. In 7 day group, positive SP fibrae became thicker, but there quantities seemed to remain the same. There was positive correlation between absorbance changes of positive SP IR products and quantity of exudation in middle ear mucosa. Middle ear NPY IR was not affected by secretory otitis media. Conclusions: SP might be the important inflammatory medium of secretory otitis media. Middle ear effusion might not be related to NPY.
