摘要
目的评价以紫杉醇（TAX）联合顺铂（DDP）、亚叶酸钙（CF）、5-氟尿嘧啶（5-Fu）持续滴注治疗晚期胃癌的疗效。方法39例均为经病理学证实的胃癌患者，接受以TAX为主的联合化疗：TAX135mg／m^2，静脉滴注3h，第1天；DDP30mg／m2＋生理盐水500ml，静脉滴注，第2～4天；CF200mg／m^2＋5％葡萄糖注射液250ml，静脉滴注2h，第2—3天；5-Fu400mg／m^2静脉推注10min，再接5-Fu2g／m^2用输液泵连续滴注48h，21d为1周期。所有患者至少接受2个周期治疗。结果39例患者中除1例因过敏反应停药外，其余38例CR3例，占7．8％，总有效率55．2％。主要毒副反应为骨髓抑制、恶心呕吐、腹泻、口腔黏膜炎、脱发和关节肌肉痛，但均可耐受。结论TAX、DDP联合CF＋5-Fu持续静脉滴注治疗晚期胃癌有较好的临床治疗效果，毒性可以耐受。
Objective To observe the efficacy and toxicity of paclitaxel（TAX） plus cisplatin （CDDP） combind with leucovorin/5-fluorouracil continuous infusion in patients with advanced gastric cancer. Method 39 patients with pathologic diagnosis of advanced gastric cancer were observed. The chemotherapy regimen comprised of TAX 135 mg/m^2 infusion for 3 hours, days 1, DDP 30 mg/m^2 infusion days 2 to 4, CF 200 mg/m^2 infusion for 2 hours followed by 10 minutes intravenous infusion of 5-Fu 400 mg/m^2, then 48 hours infusion of 5-Fu 2.0 g/m^2 with an ambulatory pump, repeated every 3 weeks. All patients were received at least two cycles of treatment. Results One patient was stoped chemotherapy because of allergic reaction, 38 patients could be evaluated. The total response rate was 55.2 %. Three obtained complete remission（CR） （7.8 %）. The major toxic side effects were neutropenia, nausea, vomiting, mucositis, alopecia, arthralgia and myalgia, but were tolerable. Conclusions The combination of paclitaxel and cisplatin and 5-Fu continuous infusion is an effective and tolerable regimen in treatment of gastric cancer.
